Yr Athro Emeritws mewn Llawfeddygaeth Llafar a Maxillofacial
Trosolwyg
Rwy'n Athro Emeritws Llawfeddygaeth y Geg a Maxillofacial, ac yn Athro Emeritws yn Sefydliad Arloesi Diogelwch, Troseddu a Cudd-wybodaeth y Brifysgol a gyd-sefydlais yn 2015.
Mae fy ymchwil ar benderfyniadau clinigol, trais cymunedol a'r ecosystem dystiolaeth wedi gwneud llawer o gyfraniadau i bolisi clinigol a chyhoeddus ac i ddeddfwriaeth. Wedi'i ysgogi gan fy darganfyddiadau, arweiniais y gwaith o ddatblygu partneriaeth diogelwch cymunedol prototeip a gadeiriais rhwng 1997-2017 ac a ddefnyddiwyd fel model yn Neddf Trosedd ac Anhrefn 1998 a oedd yn gorfodi creu partneriaethau o'r fath ledled Prydain Fawr.
Mae'r arloesiadau yn cynnwys Sefydliad Gwyddoniaeth yr Heddlu Prifysgolion yng Nghymru, y "Model Caerdydd" rhannu gwybodaeth ar gyfer atal trais a fabwysiadwyd yn strategaeth lleihau trais y DU yn 2008, gan y Llywodraeth Glymblaid yn 2010 a chan Lywodraethau dilynol y DU, a llwybr gofal cynhwysfawr i bobl sy'n cael eu niweidio gan drais. Mae Model Caerdydd wedi cael ei gymeradwyo gan WHO a Chanolfannau ffederal yr Unol Daleithiau ar gyfer Rheoli ac Atal Clefydau a gyhoeddodd ei becyn cymorth Model Caerdydd ac sy'n ariannu ac yn rheoli Rhwydwaith Cenedlaethol Caerdydd yr Unol Daleithiau o 20+ o ddinasoedd ac adrannau iechyd cyhoeddus y wladwriaeth sy'n gweithredu'r Model. Mae gweithredu a gwerthuso yn Awstralia a Jamaica a gwledydd eraill yn parhau.
Arweiniodd arbrofion maes fy nhîm ac eiriolaeth ar anafiadau gwydr at newid i lestri gwydr caled a polycarbonad a llestri plastig yn y fasnach drwyddedig yn y DU. Dechreuais ac rwy'n awdur safonau ymarfer a gyhoeddwyd gan bedwar Coleg Brenhinol meddygol.
Derbyniwyd fy nghynnig yn 2008 ac eiriolwr dros gorff proffesiynol annibynnol ar gyfer plismona gan yr Ysgrifennydd Cartref a lansiwyd y Coleg Plismona newydd yn 2013 wedi'i ategu gan ddeddfwriaeth sylfaenol. Mabwysiadwyd fy nghynnig ar gyfer sefydliad tebyg sy'n gosod safonau ar gyfer prawf - y Sefydliad Prawf a lansiwyd yn 2024 - hefyd a'i gyd-ariannu gan y llywodraeth. Roeddwn i'n allweddol yn sefydlu'r Coleg Addysgu newydd, y sefydliad sefydlog annibynnol ar gyfer athrawon ysgol ac addysgu 0-19, y gwasanaethais fel ymddiriedolwr sylfaenol a chymrawd y Coleg. Ysbrydolwyd y tri sefydliad cenedlaethol newydd hyn i gyd gan fodel meddygol y Coleg Brenhinol.
Ysgogodd fy ymchwil a'm eiriolaeth ganllawiau ac arfarniad technoleg cyntaf y Sefydliad Cenedlaethol ar gyfer Iechyd a Rhagoriaeth Glinigol (NICE) - a gyhoeddwyd yn 2000. Enillais Wobr Troseddeg Stockholm 2008 (y cyntaf yn y DU a dderbyniodd yr hyn a ddisgrifiodd The Times fel "cyfwerth â gwobr Nobel"); rwy'n Gymrawd Anrhydeddus Coleg Brenhinol y Seiciatryddion sy'n adlewyrchu fy nghyfraniadau i wasanaethau triniaeth alcohol a straen trawmatig; o'r Coleg Meddygaeth Frys; ac o Gyfadran Iechyd y Cyhoedd y Colegau Brenhinol y Meddygon.
Rwyf wedi gwasanaethu fel aelod etholedig o'r cyngor ac ymddiriedolwr Coleg Brenhinol Llawfeddygon Lloegr a ddyfarnodd FRCS Anrhydeddus i mi yn 2012, fel Is-Ddeon y Gyfadran Llawfeddygaeth Ddeintyddol, ac fel is-gadeirydd yr elusen genedlaethol Victim Support. Cyfrannais at y Ddeddf Trais Domestig (Cymru) a siaradais ar ran y GIG yn lansiad y Papur Gwyn yn 2012.
Arweiniodd fy nghynigion ymchwil a pholisi ar yr hyn a ddiffinais fel yr ecosystem dystiolaeth yn uniongyrchol at sefydliadau rhagoriaeth newydd - What Works Centres sy'n cyfateb i NICE - mewn addysg, lleihau troseddu a sectorau eraill, at y Cyngor Gwaith yn Swyddfa Cabinet y DU yr oeddwn yn aelod annibynnol ohono o'i sefydlu hyd at 2020, at Gronfa Gwybodaeth yr Heddlu (sy'n cyfateb i'r Sefydliad Cenedlaethol ar gyfer Ymchwil Iechyd) ac at Banel Treialon ESRC - sy'n cyfateb i Uned Treialon MRC. Dechreuais y gyfres uwchgynhadledd proffesiynau a chynnull uwchgynhadledd 2013 ar ymarfer tystiolaeth; cychwynnodd a drafftiodd Ddatganiad Tystiolaeth 2017 a lofnodwyd yn y Gymdeithas Frenhinol gan lywyddion holl Golegau Brenhinol y DU ac Iwerddon, y Coleg Plismona a'r Coleg Addysgu Siartredig; a chychwynnodd a chynnull Symposiwm Balliol ar Dystiolaeth 2022, digwyddiadau i gyd dan gadeiryddiaeth yr Arglwydd O'Donnell, cyn Ysgrifennydd y Cabinet.
Er mwyn dathlu ac ysgogi effaith iresearch, dechreuais a dod o hyd i gyllid ar gyfer cynllun Athro Bazalgette y DU a sefydlwyd gan y Gyfadran Iechyd Cyhoeddus yn 2017.
Arweiniais a chyd-arwain cyfres o hap-dreialon o gyngor byr ar alcohol (cyfweliadau ysgogol) mewn clinigau trawma, damweiniau ac achosion brys, gwasanaethau prawf, llysoedd ynadon a gofal sylfaenol ac, wedi'i ysgogi gan y dystiolaeth newydd a gynhyrchwyd, cychwynnais ac arweiniais ei weithredu yn y GIG yng Nghymru a Lloegr, a ariannwyd gan ddwy Bartneriaeth Trosglwyddo Gwybodaeth UKRI. Roeddwn i'n aelod o grŵp strategaeth alcohol llywodraeth y DU a ddatblygodd a chyflwynodd y strategaeth genedlaethol ddiweddaraf a oedd yn cynnwys polisi newydd sy'n deillio o ymchwil fy nhîm.

Mae fy nghanfyddiadau ymchwil wedi'u mabwysiadu yn Neddf Trwyddedu 2003, yn Strategaethau Lleihau Niwed Alcohol 2007 a 2012 a chan Gomisiwn y Gyfraith. Fel sylfaenydd, Ymddiriedolwr ac Is-gadeirydd (2001-2004) yr elusen genedlaethol Victim Support arweiniais y gwaith o ddatblygu gwasanaethau integredig i ddioddefwyr.
Roedd fy ngwaith ar gyfer Comisiwn Brenhinol ar Gyfiawnder Troseddol 1993 yn cynnwys golygyddol BMJ ar rôl tystiolaeth arbenigol mewn achosion troseddol. Rwy'n cynrychioli Cymru yng Nghynghrair Atal Trais Sefydliad Iechyd y Byd ac wedi gwasanaethu fel aelod o lawer o bwyllgorau a grwpiau cyfeirio'r llywodraeth gan gynnwys seminarau polisi dan gadeiryddiaeth Ysgrifenyddion Cartref olynol.
Effaith ymchwil
Mae fy ymchwil wedi gwneud llawer o gyfraniadau i bolisi a deddfwriaeth glinigol a chyhoeddus. Arweiniais y Grŵp Ymchwil Penderfyniadau Clinigol (1991-1999), a gynhyrchodd fwy na 40 o gyhoeddiadau yn ymwneud â gwneud penderfyniadau mewn llawfeddygaeth, deintyddiaeth ac anesthesia. Ysgogodd ganllaw dylanwadol Coleg Brenhinol Llawfeddygon Lloegr a gyhoeddwyd ym 1996 ac arfarniad technoleg cyntaf hanesyddol y Sefydliad Cenedlaethol dros Ragoriaeth Glinigol (NICE) ar dynnu trydydd molars (dannedd doethineb) proffylactig yn 2000.
O ganlyniad i'r canllawiau hyn a'm eiriolaeth dros lawdriniaeth ddewisol, gostyngodd tynnu trydydd molar yn sylweddol iawn: cynhaliwyd 30,000 yn llai o lawdriniaethau trydydd molar yng ngwasanaethau deintyddol y GIG yn 2003 o'i gymharu â 1996.
Sefydlodd fy ymchwil ar gamddefnyddio alcohol gysylltiad achosol rhwng pyliau yfed ac anaf mewn ymosodiad, yn rhannol trwy ymchwil a datblygu y GIG ac arbrofion ar hap o ymyriadau byr (ysgogol) ym mywydau dioddefwyr a throseddwyr. Datblygais ymyrraeth camddefnyddio alcohol un stop cost-effeithiol/gofal clinig trawma, sydd bellach yn destun deunyddiau hyfforddi a gweithredu "Have a Word" ledled Cymru a Lloegr.
Mae fy nghanfyddiadau ymchwil hefyd yn cynnwys cysylltiadau cryf rhwng prisiau alcohol ac anaf a gafwyd mewn trais yng Nghymru a Lloegr; bod cyfradd yr anafiadau ymosodiad yng Nghymru a Lloegr wedi aros yn sefydlog rhwng 1995-2000 ac yna wedi gostwng yn sydyn - tuedd sy'n parhau hyd at 2013; ac y gall defnyddio data unigryw o adrannau brys ysbytai leihau trais mewn dinasoedd yn gost-effeithiol.
Arweiniodd fy ymchwil gyda Michael Harrison ar ddylunio helmedau beicio at ddatblygu, cynhyrchu a gwerthu manwerthu'r helmed FaceSaver patent, a lansiwyd gan y gyrrwr Fformiwla Un David Coulthard yn Sioe Moduron Genedlaethol 2002.
Yn seiliedig ar fy ymchwil ar wasanaethau cyhoeddus, cynigiais sefydlu Coleg Brenhinol Plismona (a sefydlwyd yn 2013) ac ysgolion heddlu prifysgol fel sylfaen ar gyfer gwasanaethau heddlu.
Rwy'n eiriolwr brwd o werthuso trylwyr mewn gwasanaethau cyhoeddus, er enghraifft trwy ddatblygu uned treialon maes y Cyngor Ymchwil Economaidd a Chymdeithasol (ESRC) (a sefydlwyd yn 2014), trwy sefydlu cyfwerth â NICE mewn lleihau troseddu ac addysg (y canolfannau "What Works" a gyhoeddwyd yn 2013); a thrwy ddatblygu rolau ymarferwyr-academyddion ar draws gwasanaethau cyhoeddus.
Roeddwn i'n allweddol wrth sefydlu'r Sefydliad Prawf. Fel ysgrifennydd sylfaenol a chadeirydd y Gymdeithas Academaidd arbenigol genedlaethol hon, arloesais raglen hyfforddi glinigol/academaidd integredig a ddaeth yn fodel yn y DU ar draws gwyddoniaeth feddygol. Rwyf wedi gwasanaethu fel llywydd fy nghymdeithas ymchwil ryngwladol arbenigol.
Bywgraffiad
Ar ôl cwblhau fy hyfforddiant clinigol yn Ysbyty Coleg y Brenin, graddiais o Brifysgol Llundain ym 1973. Ar ôl penodiadau Tŷ ac Uwch Swyddog Tŷ yn Kings, Ysbyty y Frenhines Victoria East Grinstead ac Ysbyty Deintyddol Eastman yn Llundain, cefais fy mhenodi'n Gofrestrydd mewn Llawfeddygaeth y Geg a Maxillofacial yn Rhydychen lle cefais Gymrodoriaeth Ymchwil yn Adran Llawfeddygaeth Nuffield.
Sefydlodd ymchwil ar gyfer fy MSc Rhydychen fod diffyg creithiau ar ôl anaf tymheredd isel yn adlewyrchu cadwraeth y matrics allgellog a bod iachâd trwy fwriad cynradd ac eilaidd yn cynrychioli continwwm yn hytrach na phrosesau amlwg gwahanol.
Cwblheais fy hyfforddiant llawfeddygol arbenigol yn Ysbytai Prifysgol Leeds lle aeth apwyntiad Gweinyddiaeth Datblygu Tramor Llywodraeth y DU (bellach Swyddfa Dramor, y Gymanwlad a Datblygu) â mi i Ysbyty Athrofaol Ahmadu Bello yn Nigeria lle ymchwiliais i gysylltiad firws Epstein Barr ag ameloblastoma. Cyhoeddwyd y canfyddiadau hyn yn y cyfnodolyn Cancer.
Sylwadau bod streiciau y glowyr ym Maes Glo Swydd Efrog yn yr 1980au yn gysylltiedig â mwy o drais cymunedol ysgogodd fy PhD, "Assault: characteristics of wound and injuries", yn ystod fy mhenodiad fel Uwch Ddarlithydd ac Ymgynghorydd Llawfeddyg Llafar a Maxillofacial ym Mhrifysgol Bryste.
Mae'r ymchwil ryngddisgyblaethol hon, sy'n cwmpasu'r gwyddorau cymdeithasol a meddygol, wedi darparu llawer o fewnwelediadau newydd, yn enwedig i ba raddau y mae trais sy'n arwain at driniaeth y GIG yn cael ei adrodd i'r heddlu a'r effaith sylweddol ar iechyd meddwl ar yr anafedig - anhwylder ôl-drawmatig a chyflyrau eraill. Cyhoeddwyd y darganfyddiadau hyn mewn cyfnodolion meddygol a gwyddorau cymdeithasol blaenllaw ac fe wnes i grynhoi goblygiadau ar gyfer gwasanaethau clinigol ac atal trais mewn cyfres o 10 golygyddol BMJ a Lancet sydd wedi helpu i lywio fy ymchwil ac arloesedd ers hynny.
Rwy'n byw gyda fy nheulu yng ngogledd deiliog Caerdydd, lle, gydag eraill sydd â diddordeb yn y Chwyldro Diwydiannol yng Nghymru, adeiladais ffwrnais chwyth weithiol.
