Athro Anrhydeddus. Orthogeriatregydd Ymgynghorol yn Ysbyty Athrofaol Cymru
Trosolwyg
Bob blwyddyn mae 80,000 o bobl yn torri eu cluniau yn y DU. Mae'r rhan fwyaf ohonynt dros 80 oed ac mae ganddynt amrywiaeth o broblemau cymdeithasol, meddygol a seicolegol sy'n 'fregus'.
Er gwaethaf llwyddiant llawfeddygaeth fodern ac anesthesia, mae llawer yn wynebu arosiadau hir yn yr ysbyty a dibyniaeth hirdymor, sy'n golygu bod yr un cyflwr hwn yn costio dros £2 biliwn y flwyddyn i wasanaethau iechyd a chymdeithasol.
Mae fy 30 mlynedd yn gofalu am bobl sydd â thoriadau clun wedi fy helpu i ddeall y gofal, tosturi a chydweithio sydd eu hangen ar bob person bregus a hŷn yn yr ysbyty, wrth i mi archwilio mewn set o fideos ar dab 'Addysgu' y dudalen we hon.
Mae fy nhabl 'Ymchwil' yn dangos sut rwyf wedi adeiladu ar fy ngwaith clinigol; gan ddefnyddio epidemioleg, treialon ac archwiliad cenedlaethol i fodelu sut y dylai gwasanaethau gofal cleifion ac iechyd ledled y byd newid i gwrdd â her poblogaeth sy'n heneiddio.

Addysgu
Rwyf wedi defnyddio darlithoedd ac addysgu rhyngweithiol i hyrwyddo'r arbenigedd newydd o orthogeriatrics yn fy ysbyty, yng Nghymru, ledled y DU ac yn rhyngwladol. Mae fy addysgu o fyfyrwyr, llawfeddygol a meddygol iau a'n tîm amlddisgyblaethol, a'm darlithio cenedlaethol a rhyngwladol yn anelu at gefnogi mabwysiadu archwiliad torri cluniau a chydweithio orthopedig-geriatreg mewn gwledydd eraill.
Mae protocolau ar gyfer asesu a rheoli cleifion yn allweddol i wella gofal a chanlyniad. Mae llawer o'm gwaith wedi canolbwyntio ar ddatblygu protocolau ar gyfer gofal cychwynnol fy nghleifion fy hun gyda chlun a thoriadau femoral eraill.
Crynhoir y protocolau hyn yn y ddogfen clercio a 'bwndel gofal' y trefnir gofal yn ei gylch yng Nghaerdydd:

Mae fy nghyfres o sgyrsiau YouTube yn adnodd sefydlu ar gyfer myfyrwyr, nyrsys, therapyddion a phobl iau sy'n ymuno â'r adran. Mae'r sgyrsiau hyn hefyd wedi'u bwriadu fel adnodd am ddim i bobl sy'n datblygu gwasanaeth torri clun mewn gwledydd lle mae nifer yr achosion o'r anaf hwn yn codi, ond mae arbenigeddau orthogeriatrig a meddygaeth geriatreg wedi'u datblygu'n llai da.

Bywgraffiad
Yn y 1990au arloesodd geriatregwyr yng Nghaerdydd gefnogaeth feddygol cleifion bregus a hŷn ein wardiau trawma acíwt, ac yn 1997 fi oedd y geriatregydd ymgynghorol cyntaf yn y byd i weithio'n llawn amser yn y lleoliad orthopedig acíwt.
Mae cefnogaeth hynod hael cydweithwyr Geriatreg, nyrsys arbenigol, fferyllwyr ac eraill wedi fy ngalluogi i helpu i sefydlu'r arbenigedd newydd o 'orthogeriatrics', a phrofiad o gydweithio â llawfeddygon yng Nghaerdydd at fy nghyd-awduro 'Llyfr Glas' Cymdeithas Orthopedig Prydain ar doriadau breuder, a chanllawiau NICE 2011 ar dorri clun.
Rwyf wedi arwain yr archwiliad clinigol cenedlaethol o dorri clun, y Gronfa Ddata Genedlaethol Torasgwrn Clun (NHFD ) ers 2013, ac wedi cynnal archwiliadau sbrint cenedlaethol mewn cydweithrediad â Chymdeithas Anaesthesia (ASAP) a Chymdeithas Siartredig Ffisiotherapi (PHFSA).
Ers ei sefydlu, mae'r NHFD wedi llywyddu dros haneru marwolaethau, gan ddefnyddio gwefan mynediad agored i helpu staff clinigol, rheolwyr gwasanaethau iechyd a'r cyhoedd yn gyffredinol i archwilio a gwella ansawdd y gofal a gynigir mewn gwahanol ysbytai; rhywbeth rwy'n ei drafod yn fy rôl fel Arweinydd Geriatreg ar gyfer y NHFD yn y fideo hwn.
Fel cadeirydd grŵp diddordeb arbennig Hip Fracture Audit o'r Rhwydwaith Torri Fragility byd-eang (FFN). Rwyf bellach yn ceisio cefnogi mentrau tebyg ledled y byd. Gall gwasanaethau iechyd mewn gwahanol wledydd ddysgu oddi wrth ei gilydd os bydd pob archwiliad cenedlaethol yn ceisio casglu data tebyg; y 'set ddata gyffredin leiaf'; rhywbeth rwy'n ei drafod yn y fideo hwn.
