Mae fy ymchwil yn archwilio elfennau sefydliadol ymarfer gofal iechyd, gyda ffocws penodol ar waith nyrsys, ansawdd a diogelwch gofal iechyd, a chydlynu gofal.
Dros yrfa ymchwil sy'n rhychwantu 34 mlynedd, wedi gweithio gyda chydweithwyr cenedlaethol a rhyngwladol ar draws disgyblaethau - gan gynnwys meddygaeth, deintyddiaeth, seicoleg, cymdeithaseg, mathemateg, ac economeg iechyd - i adeiladu rhaglen sylweddol o ymchwil. Thema ganolog yn fy ymchwil yw gwaith trefnu anweledig nyrsys - y cydlynu, blaenoriaethu, a llafur perthynol sy'n sail i ofal diogel ac effeithiol ond sy'n aml yn mynd heb ei gydnabod mewn systemau ffurfiol. Dechreuwyd y rhaglen ymchwil hon trwy astudiaeth ethnograffig sylfaenol, gyda chefnogaeth Cymrodoriaeth Gwyddoniaeth Gwella Sefydliadau Iechyd, a archwiliodd arferion trefnu nyrsys a'u rôl wrth gynnal systemau gofal. Un o ganlyniadau allweddol yr astudiaeth hon oedd datblygu Theori Mobileiddio Drosiadol (TMT), fframwaith cymdeithasegol ar gyfer dadansoddi gweithredu ar y cyd mewn lleoliadau sefydliadol cymhleth (https://www.translationalmobilisationtheory.org). Mae TMT yn ddamcaniaeth sylfaenol sy'n darparu lens gysyniadol ar gyfer deall cydlynu mewn prosesau sefydliadol sy'n dod i'r amlwg.
Mae'r ymchwil hon yn gosod y sylfaen ar gyfer datblygu'r Pecyn Cymorth TRACT, adnodd digidol a gynlluniwyd i gefnogi rheoli a mesur cyfraniadau sefydliadol nyrsys a gwerslyfr, Care Trajectory Management: Foundations for Organizing Work in Nursing—y cyntaf o'i fath i fynd i'r afael â chymhwysedd sefydliadol mewn nyrsio.

Diogelwch Cleifion a Systemau Sefydliadol
Mae diogelwch cleifion yn bryder canolog mewn polisi ac ymarfer gofal iechyd, ac mae fy ymchwil yn mynd i'r afael â'r ffactorau sefydliadol a gweithlu sy'n siapio canlyniadau. Rwy'n canolbwyntio ar sut mae barn broffesiynol nyrsys, gwaith cydlynu, ac ymyriadau ar lefel system yn cyfrannu at amgylcheddau gofal mwy diogel. Mae astudiaethau diweddar wedi canolbwyntio ar systemau rhybuddio cynnar pediatrig a staffio nyrsio diogel. Mae'r ddau yn ddamcaniaethol wedi'u llywio gan Theori Mobileiddio Cyfieithuol.
Systemau Rhybuddio Cynnar a Diogelwch Pediatrig
Ymchwiliodd yr astudiaeth PUMA a ariennir gan NIHR (System rhybudd cynnar pediatrig Utilisation and Morbidity Avoidance) i ddatblygu, gweithredu a gwerthuso rhaglen wella system gyfan ar gyfer systemau rhybuddio cynnar pediatrig. Wedi'i gynnal mewn pedwar ysbyty, cyfunodd yr astudiaeth ddadansoddiad cyfres amser torri gydag astudiaethau achos ethnograffig i asesu sut mae amodau sefydliadol yn effeithio ar ganfod ac ymateb i ddirywiad clinigol mewn plant.
Tynnodd canfyddiadau sylw at gyfyngiadau offer tracio a sbarduno ar wahân a phwysleisiodd yr angen am dulliau systemau cyfan o ddiogelwch. Mae'r astudiaeth yn cynnig fframwaith ar gyfer gwella system barhaus ac mae ganddo oblygiadau ar gyfer ymarfer clinigol, polisi, ac ymchwil yn y dyfodol.

Staffio Nyrsys Diogel
Roedd yr astudiaeth Pro-Judge, a ariennir gan Sefydliad RCN, yn archwilio sut mae nyrsys yn defnyddio barn broffesiynol wrth benderfynu staffio ar draws chwe ysbyty yng Nghymru a Lloegr. Nod oedd agor y "blwch du" o systemau staffio nyrsio ac arweiniodd at ddatblygu CUSIAT, offeryn sy'n helpu nyrsys i fynegi amodau cyd-destunol cymhleth sy'n effeithio ar anghenion staffio.

Mae'r gwaith hwn yn ategu fy ymchwil ehangach i elfennau sefydliadol gwaith nyrsio, gan gynnwys y llafur anweledig y mae nyrsys yn eu gwneud i gynnal systemau gofal. Mae'n cyd-fynd â datblygiad Pecyn Cymorth TRACT, sy'n cefnogi rheoli a mesur cyfraniadau trefnu nyrsys ar gyfer cynllunio'r gweithlu.
Cydlynu Gofal ac Integreiddio Gwasanaethau
Mae cydlynu gofal yn ffocws hirsefydlog fy ymchwil, gan gwmpasu'r gwaith perthynol, sefydliadol a systemig sydd ei angen i gysylltu gwasanaethau a chefnogi cleifion ar draws ffiniau gofal. Mae fy astudiaethau wedi archwilio sut mae rolau a chyfrifoldebau yn cael eu trafod ar draws iechyd a gofal cymdeithasol, sut mae pontio yn cael eu rheoli rhwng gwasanaethau pediatrig ac oedolion, a sut mae llwybrau gofal yn cael eu hadeiladu a'u gweithredu yn ymarferol.
Roedd gwaith cynnar yn archwilio'r rhaniad llafur mewn iechyd a gofal cymdeithasol mewn cleifion a oedd wedi dioddef strôc acíwt gyntaf. Dilynwyd hyn gan astudiaeth ar y trawsnewid o wasanaethau diabetes pediatrig i wasanaethau diabetes oedolion, a amlygodd heriau parhad a hunaniaeth ar draws ffiniau sefydliadol. Archwiliodd ymchwil ddilynol ar lwybrau gofal eu defnydd fel technolegau cydlynu a rheoli, gan ddatgelu eu potensial a'u cyfyngiadau wrth lunio teithiau cleifion.
Yn fwyaf diweddar, ymchwiliodd astudiaeth Making Connection - a gynhaliwyd fel rhan o Gymrodoriaeth Gwyddoniaeth Gwella Sefydliad Iechyd - i'r arferion cymdeithasol-ddeunydd y mae cleifion yn cael eu gwneud a gofal yn cael ei mobilized. Mae'r ymchwil hon yn sail i'm llyfr sydd ar ddod, Beyond Pathways: Making Patients and Mobilising Care, sy'n cynnig fframwaith cysyniadol newydd ar gyfer deall gwaith cydlynu mewn systemau gofal iechyd cyfoes.

Rwy'n gymdeithasegydd trefnu a darparu gofal iechyd, gyda diddordebau ymchwil mewn nyrsio, cysylltiadau cymdeithasol-dechnegol mewn systemau gwaith, a gwella gwasanaethau. Dros y blynyddoedd, rwyf wedi cydweithio â chydweithwyr ar draws meddygaeth, deintyddiaeth, seicoleg, cymdeithaseg, mathemateg, ac economeg iechyd i adeiladu rhaglen sylweddol o ymchwil ar drefnu a darparu gofal iechyd. Rwyf wedi dal amrywiol rolau arwain uwch ac wedi cyfrannu at adeiladu gallu ymchwil mewn nyrsio, proffesiynau iechyd cysylltiedig, a gwasanaethau iechyd.
Ymunais â Phrifysgol Caerdydd ym 1996 fel Cymrawd Ymchwil yn yr Ysgol Nyrsio, yn ystod blwyddyn olaf fy PhD. Goruchwyliwyd fy ymchwil ddoethurol - astudiaeth ethnograffig o drafod ffiniau rôl nyrsio o ddydd i ddydd mewn Ysbyty Cyffredinol Dosbarth - gan yr Athro Robert Dingwall a'r Athro Veronica James ym Mhrifysgol Nottingham. Wedi'i ariannu gan ysgoloriaeth yr Adran Iechyd, roedd yn rhan o strategaeth genedlaethol i gryfhau gallu ymchwil mewn nyrsio, bydwreigiaeth, a phroffesiynau iechyd cysylltiedig. Mae'r astudiaeth hon yn gosod y sylfaen ar gyfer fy niddordeb parhaol mewn trefniadaeth gymdeithasol gwaith gofal iechyd a'm hymrwymiad i ddatblygu sylfaen ymchwil nyrsio.
