Rwy'n arwain rhaglen o ymchwil fethodolegol sy'n archwilio'r materion moesegol, cyfreithiol ac ymarferol sy'n ymwneud ag ymchwil sy'n cynnwys oedolion nad oes ganddynt ganiatâd capasiti. Oedolion sydd â galluedd diffygiol i gydsynio
Fy mhrif ddiddordeb ymchwil yw mynd i'r afael â'r heriau moesegol a methodolegol o ran cynnwys oedolion nad oes ganddynt allu mewn ymchwil. Ar hyn o bryd mae gen i Gymrodoriaeth Uwch NIHR 'Penderfynu am ymchwil i eraill: treial astudiaeth ddichonoldeb ac effeithiolrwydd o ymyriad cymorth penderfyniadau newydd i ymgyngoreion a chynrychiolwyr cyfreithiol oedolion nad oes ganddynt y gallu i gydsynio (CONSULT)' (2021-2026). Bydd yr ymyrraeth (offeryn cymorth penderfynu i wella gwneud penderfyniadau drwy ddirprwy ar gyfer ymchwil yn cael ei werthuso gan ddefnyddio dyluniad 'Astudio o fewn Treial' (SWAT) i wreiddio'r gwerthusiad mewn hyd at 5 treial cynnal.
Fel rhan o brosiect CONSULT, arweiniais astudiaeth ansoddol yn archwilio barn ymchwilwyr a gweithwyr gofal iechyd proffesiynol am y rhwystrau a'r hwyluswyr i gynnal treialon yn ymwneud ag oedolion nad oes ganddynt alluedd i gydsynio (Astudiaeth CONSULT-ENABLE ). Rwyf hefyd yn arwain yr Astudiaeth CONSULT-ADVANCE i archwilio barn rhanddeiliaid am ymarferoldeb a derbynioldeb cynllunio ymchwil ymlaen llaw, sy'n broses i bobl fynegi eu dymuniadau am gyfranogiad ymchwil cyn colli unrhyw gapasiti i gydsynio.
Rwy'n arwain y gwaith o ddatblygu canlyniad craidd a osodwyd i werthuso ymyriadau sy'n gwella penderfyniadau dirprwyol ynghylch cyfranogiad ar ran oedolion nad oes ganddynt gapasiti (Astudiaeth COnSiDER) sydd wedi'i gofrestru ar gronfa ddata COMET (#1409). Ochr yn ochr â hyn, rwy'n datblygu mesur canlyniadau newydd i asesu ansawdd y penderfyniadau a wneir am ymchwil ar ran rhywun nad oes ganddynt alluedd i gydsynio - y Raddfa Gyfunol ar gyfer Penderfyniadau Cydsyniad Gwybodus trwy Ddirprwy (graddfa CONCORD).
Rwyf hefyd yn arwain y gwaith o ddatblygu'r Fframwaith NIHR CYNNWYS Galluedd Diffygiol i Gydsynio sy'n offeryn i helpu ymchwilwyr i ddylunio treialon sy'n cynnwys oedolion â galluedd diffygiol i gydsynio. Mae pecyn cymorth gweithredu yn cael ei ddatblygu fel rhan o brosiect a gefnogir gan gyllid 'Arloesi i Bawb' gan Brifysgol Caerdydd (2022-2023).
Yn flaenorol, cynhaliais Gymrodoriaeth Ymchwil Ddoethurol NIHR i archwilio'r heriau moesegol, cyfreithiol ac ymarferol ynghylch cynnwys y poblogaethau hyn sydd heb gynrychiolaeth ddigonol, gan ddefnyddio dulliau cymysg a dulliau rhyngddisgyblaethol (2016-2019). Arweiniais astudiaeth ddilynol i gynnal profion gwybyddol ar yr ymyrraeth cymorth penderfyniadau (Astudiaeth PENDERFYNIAD 2) a ariannwyd gan Ymddiriedolaeth Wellcome trwy Gyllid Sefydliadol ISSF (2020).
Rwy'n goruchwylio nifer o fyfyrwyr meddygol BSc Rhyng-gyfrifol a phrosiectau PhD yn y maes hwn, ac yn ddiweddar curadodd ymchwil ac adnoddau ar 'Ymchwil a gallu i gydsynio' ar gyfer gwefan Gofal Cymdeithasol Cymru https://socialcare.wales/curated-research/research-and-capacity-to-consent (fersiwn Gymraeg: https://gofalcymdeithasol.cymru/curadur-yr-ymchwil/ymchwil-ar-gallu-i-gydsynio ).
Cartrefi Gofal
Mae gen i ddiddordeb arbennig mewn ymchwil cartrefi gofal. Arweiniais Astudiaeth Blaenoriaethu Ymchwil ledled y DU mewn Cartrefi Gofal, a oedd â'r nod o nodi cyfres o flaenoriaethau ymchwil ar gyfer ymchwil cartrefi gofal gyda grŵp allweddol o randdeiliaid, a chanlyniad craidd cyfredol a osodwyd ar gyfer atal COVID-19 mewn cartrefi gofal (COS COVID PCARE). Rwyf hefyd yn ymwneud â chydweithio ag ymchwilwyr/sefydliadau cenedlaethol a rhyngwladol eraill i ddatblygu cynigion ymchwil sy'n cynnwys pobl hŷn mewn gofal hirdymor, y mae rhai ohonynt yn mynd i'r afael â meysydd a nodwyd fel blaenoriaethau ymchwil.
Rwy'n gyd-ymgeisydd ar brosiect sy'n datblygu hyfforddiant cymorth hunanreoli ar gyfer gofalwyr oedolion hŷn ag osteoarthritis a chyflyrau ychwanegol (SUSTAIN).
Treialon Clinigol
Mae gweithgareddau ymchwil blaenorol yn cynnwys cyfrannu at ddylunio, cyflwyno ac adrodd ar hapdreialon rheoledig aml-ganolfan, gan gynnwys: Plant ag ecsema, Rheoli gwrthfiotigau - Astudiaeth CREAM (NIHR HTA), STeroids Llafar ar gyfer Datrys otitis cyfryngau gydag effusion (OME) Mewn Plant - Astudiaeth OSTRICH (NIHR HTA) a Probiotics i Leihau Heintiau iN CarE reSidentS cartref - PRINCES Study (NIHR EME).
Rwy'n arwain 'Prosiect Astudio o fewn A' (SWAP) sy'n astudiaeth fethodolegol sydd wedi'i hymgorffori â threial i werthuso effeithiolrwydd clinigol a chost ymyriad cymorth hunanreoli unigol osteoarthritis wedi'i gyd-ddylunio ar gyfer hyrwyddo ansawdd bywyd sy'n gysylltiedig ag iechyd a lleihau costau gofal iechyd (TIPTOE) (2023-2026). Bydd y SWAP wedi'i fewnosod yn archwilio ac yn mynd i'r afael â recriwtio a chadw grwpiau sydd heb eu gwasanaethu'n ddigonol yn y treial ac yn nodi rhwystrau i'w cynnwys, ochr yn ochr â QRI QUINTET.

Gan ddefnyddio dulliau datblygu ymyrraeth sefydledig, bydd ymyrraeth yn cael ei datblygu i gefnogi sgyrsiau am ymchwil gyda phreswylwyr cartrefi gofal a hwyluso cynllunio ymlaen llaw ar gyfer cyfranogiad ymchwil. Bydd y canfyddiadau'n cefnogi cyfleoedd i drigolion cartrefi gofal gymryd rhan mewn ymchwil, sicrhau bod eu lleisiau'n cael eu clywed, a derbyn gofal o ansawdd sy'n seiliedig ar dystiolaeth yn y dyfodol. Mae angen i gyfranogwyr treial gynrychioli'r rhai mewn cymdeithas y bwriedir iddynt elwa o ganlyniadau'r treial. Bydd y grwpiau sydd angen cynrychiolaeth fwyaf yn amrywio o dreial i dreial. Gall grŵp heb ei wasanaethu ar gyfer un math o dreial fod yn wahanol iawn i grŵp arall. Gall rhesymau dros pam nad yw grwpiau sydd heb eu gwasanaethu yn cael eu cynnwys mewn rhywfaint o ymchwil fod yn gymhleth, ond mae'n haws penderfynu a yw atebion i ddatrys rhwystrau cynhwysiant yn haws i'w penderfynu os cânt eu hystyried o ddechrau treial. Mae croestoriadedd yn mynd gam ymhellach i gydnabod y gallai fod sawl ffactor (er enghraifft ethnigrwydd a rhyw) sy'n cyfuno ac yn arwain at anfantais neu wahaniaethu. Datblygwyd ychydig o offer yn ddiweddar o'r enw Fframweithiau INCLUDE (gweler https://www.trialforge.org/trial-forge-centre/diversity/) i helpu timau treialu i feddwl sut y gallai eu penderfyniadau dylunio ei gwneud hi'n hawdd neu'n anodd i rai grwpiau gymryd rhan. Nod y prosiect PhD dulliau cymysg hwn yw darparu proses symlach i dimau treial lywio eu ffordd trwy'r Fframweithiau'n effeithlon. I wneud hyn, yn gyntaf bydd angen i ni benderfynu ble mae croestoriadoldeb rhwng gwahanol grwpiau sydd heb wasanaeth digonol yn bodoli a beth mae hyn yn ei olygu i ymchwilwyr y bydd angen iddynt ystyried y grwpiau hyn wrth ddylunio eu treialon. Yn ogystal, bydd y prosiect yn ystyried pa fathau o awgrymiadau a all alluogi ymchwilwyr i ymchwilio'n effeithiol i'r rhwystrau a'r heriau y gallai'r poblogaethau croestoriadol hyn eu cael gyda chyfranogiad. Yn olaf, bydd y prosiect yn ymchwilio pa fathau o atebion y gellid eu rhoi i ymchwilwyr i sicrhau bod y cwestiynau Fframwaith hanfodol hyn nid yn unig yn cael eu gofyn ond eu gweithredu, i sicrhau bod eu treialon yn wirioneddol gynhwysol mewn ymarfer.
