Rwy'n Uwch Ddarlithydd ac yn Ystadegydd Meddygol. Mae gen i ddiddordeb mawr yn y defnydd o fethodolegau ystadegol a methodolegau eraill i nodi a disgrifio materion iechyd a lles mewn plant o fabanod newydd i bobl ifanc yn eu harddegau ac ers 2008 dyma'r meysydd lle mae'r mwyafrif helaeth o'm hymchwil wedi'i gynnal.
Mae rhan fawr o fy ymchwil wedi bod fel rhan o grŵp sydd wedi ymchwilio i effeithiau cynaeddfedrwydd, pwysau geni isel a phwysau geni isel ar gyfer oedran beichiogrwydd ar ganlyniadau gan gynnwys gallu'r ysgyfaint yn ddiweddarach mewn bywyd, nifer yr achosion o gyflyrau anadlol fel asthma, gallu ymarfer corff a marwolaeth gyffredinol. Yn fwy diweddar mae'r gwaith hwn wedi cael ei ymestyn i edrych ar effeithiau llygredd adeg genedigaeth ac yna ar farwolaethau ac afiachedd diweddarach.
Mae un arall o fy meysydd ymchwil wedi ymwneud ag amddiffyn plant, o ran dulliau newydd o nodi cam-drin plant a sefyllfaoedd lle gallai plant gael eu hanafu'n ddamweiniol oherwydd diffyg gwerthfawrogiad rhieni o beryglon.

Er nad yw fy holl waith ymchwil wedi cynnwys gwahanol elfennau o les babanod a phlant, ac mae nifer o ddarnau o waith gyda'u cyhoeddiadau cysylltiedig wedi'u crynhoi isod.
Effeithiau cynaeddfedrwydd a phwysau geni isel ar Spirometry
Dangosodd ymchwiliadau o effeithiau cael eu geni yn ystod y tymor gyda Arafu Twf Mewnwythol (IUGR), hy yn y 10% ysgafnaf ar gyfer oedran beichiogrwydd wrth gymharu â rheolaethau wedi'u pwysoli'n briodol fod gan blant a anwyd yn IUGR o garfan ALSPAC fesurau swyddogaeth ysgyfaint sylweddol is (FEV1, FVC a FEF2575) yn yr oedran ysgol na'r rheolaethau. Wrth edrych yn union ar blant yr IGUR fe ddangoson ni hefyd fod y rhai a oedd yn "dal i fyny", h.y. wedi dod yn bwysau priodol ar gyfer oedran erbyn 8-9 oed â mesurau swyddogaeth ysgyfaint uwch - er nad yn sylweddol - na'r rhai nad oeddent. Gallai'r ddau grŵp hyn gynrychioli'r plant hynny sy'n enetig fach a'r rhai a oedd yn fach adeg eu geni oherwydd rhyw reswm niweidiol h.y. ysmygu mamol.

Er mwyn ymchwilio i gynaeddfedrwydd, cymharom swyddogaeth yr ysgyfaint yn 8-9 a 14-17 oed ar gyfer plant a anwyd yn hwyr cyn-dymor (beichiogrwydd 33-34 a 35-36 wythnos) â phlant o oedran tebyg a anwyd yn ystod y tymor (>=37 wythnos beichiogrwydd). Fe wnaethom hefyd gymharu'r rhain â phlant cyn-dymor iawn a anwyd yn 25-32 wythnos beichiogrwydd. Dangosodd y canlyniadau fod gan y plant a anwyd rhwng 33 a 34 wythnos yr un peth i'r 25-32 wythnos, sy'n sylweddol dlotach swyddogaeth yr ysgyfaint yn 8-9 oed na'r term plant ond ni wnaeth y 35-36 o bobl sy'n wythnosol. Cafodd y difrod hwn yn 8-9 eu lliniaru rhywfaint gan 14-17 mlynedd.

Cafodd effaith pwysau geni isel plant tymor yn 8-9 oed ac eto yn 14-17 oed ei asesu eto gan ddefnyddio'r garfan ALSPAC. Modelwyd mesurau swyddogaeth ysgyfaint safonol, wedi'u haddasu ar gyfer rhyw, uchder ac oedran, o ran pwysau geni z-sgôr a'u haddasu ar gyfer rhyw yn ogystal â beichiogrwydd a chylchedd pen a chyplyddion posibl eraill. Roedd mesurau swyddogaeth yr ysgyfaint (FEV1, FVC a FEF2575) yn gysylltiedig yn sylweddol ar 8-9 mlynedd gyda phwysau geni a pharhaodd y perthnasoedd hyn i raddau helaeth ar ôl addasu'r modelau'n llawn. O garfan o blant cyn-dymor a thymor (n = 4284 a 2865) ymchwiliwyd i ganlyniadau a gafwyd o holiadur anadlol gan ddefnyddio modelau atchweliad wedi'u haddasu. Canfuom fod twf ffetws carlam yn ystod y trimesters 1af a'r 2il yn gysylltiedig â mwy o wheeze‐ever mewn plant a anwyd cyn y tymor. Hefyd roedd ennill pwysau babanod cyflym yn gysylltiedig â mwy o wheeze-erioed gyda phlant a anwyd ≤32 wythnos beichiogrwydd yn dangos cynnydd pwysau cyflym gyda risg uwch bum gwaith o wheeze-erioed o'i gymharu â thymor a anwyd heb ennill pwysau.

Mae ymchwilio i gleisio ar blant ifanc yn chwarae rhan bwysig wrth benderfynu a allai'r plentyn fod wedi bod yn destun camdriniaeth ai peidio. Astudiaethau poblogaeth – Pwysau geni isel a llygredd
Mae pwysau geni isel (LBW) yn gysylltiedig â marwolaethau cynyddol yn eu babandod, ond mae ei gysylltiad â marwolaethau yn ystod plentyndod a glasoed diweddarach yn llai eglur. Cynhaliwyd astudiaeth poblogaeth o'r holl enedigaethau byw yng Nghymru a Lloegr o 1993 i 2011 (12,355,251 o enedigaethau byw), gan edrych ar farwolaethau a'i brif achosion ar gyfer gwahanol grwpiau pwysau geni hyd at 18 oed. Dangosom trwy atchweliad Cox fod pwysau geni isel eithafol a chymedrol yn gysylltiedig â marwolaethau cynyddol ar bob pwynt hyd at 18 oed ac roeddem hefyd yn gallu defnyddio codau ICD9/10 i weld yr amrywiad mewn achosion marwolaeth gyda'r bandiau pwysau geni.

Gwnaethom ymchwilio i weld a yw llygredd (PM10, SO2 a NO2) yn gysylltiedig yn wahaniaethol â marwolaethau babanod, babanod newydd-anedig neu ôl-enedigol ac o fewn y rhain, achosion penodol o farwolaethau babanod ar gyfer 7,984,366 o enedigaethau byw rhwng 2001 a 2012 yng Nghymru a Lloegr. Cafodd y 36,485 o farwolaethau babanod (25,110 o fabanod newydd-anedig ac 11,375 o ôl-enedigaeth) eu modelu gydag addasiadau ar gyfer y prif gyd-sylfaenwyr canlynol: amddifadedd, pwysau geni, oedran mamol, rhyw, a genedigaeth luosog. Canfuom fod marwolaethau babanod wedi cynyddu'n sylweddol gyda NO2, PM10, a SO2 pan gymharwyd pumawd llygryddion uchaf ac isaf; fodd bynnag, roedd marwolaethau newyddenedigol yn gysylltiedig yn sylweddol â SO2 ond nid oedd yn gysylltiedig yn sylweddol â NO2 a PM10. Roedd marwolaethau ôl-enedigol yn gysylltiedig yn sylweddol â'r holl lygryddion. Ar gyfer achosion penodol, roedd pob llygrydd yn gysylltiedig yn yr un modd ag achosion endocrin marwolaethau babanod, fodd bynnag, fel arall roeddent yn gysylltiedig yn wahaniaethol ag achosion penodol eraill: Roedd NO2 a PM10 yn gysylltiedig â chynnydd mewn marwolaethau babanod o gamffurfiadau cynhenid y systemau nerfol a gastroberfeddol tra bod SO2 yn gysylltiedig â chynnydd mewn marwolaethau babanod o achosion amenedigol ac o gamffurfiadau'r system gylchrediadol.

Methodoleg newydd – siartiau twf
Mae'r siartiau twf cyfredol yn gweithio ar y rhagosodiad bod twf mewn utero yn digwydd yn yr un modd â thwf ôl-enedigol mor effeithiol i blant cynamserol mae'r twf "coll" mewn utero yn cael ei fodelu yr un ffordd â thwf ôl-enedigol. Nid yw hyn yn wir ac felly fe wnaethom gynnwys dau derm yn ein model, un yn cynrychioli cynnydd pwysau o'r geni ond mae'r llall yn cynrychioli cynnydd pwysau o'r beichiogi. Pan brofwyd ein siartiau twf disgwyliedig newydd gyda data twf gwirioneddol ar gyfer plant a anwyd hyd at 31 wythnos beichiogrwydd, canfuwyd eu bod yn sylweddol fwy cynrychioliadol na'r siartiau presennol.

Mewn papur o 2018, gwnaethom ddefnyddio dulliau atchweliad i brofi'r rhagdybiaeth, mewn newydd-anedig ar awyru mecanyddol, y gellir cyfuno mynegai nodweddion cyfradd y galon (HRCi) â model clinigol ar gyfer rhagweld canlyniadau tiwbio (neu aildiwbio) mewn babanod newydd. Gwnaethom ddangos bod modelau aml-effeithiau llinol llinol a ddefnyddir i greu modelau rhagfynegi, gan ddefnyddio newidynnau perthnasol, wedi gwella'r ddealltwriaeth o'r siawns o lwyddiant tiwbio yn sylweddol.

Deellir effaith negyddol cynaeddfedrwydd ar swyddogaeth resbiradol trwy gydol plentyndod ond roedd effeithiau cael eu geni yn y Tymor Cynnar (37-38 wythnos) yn aneglur. Fel rhan o astudiaeth holiadur o blant 1-10 oed, roeddem yn gallu cymharu 545 o enedigaethau tymor cynnar gyda 2,300 yn enedigaethau tymor llawn. Roedd gan blant a aned yn y tymor cynnar gyfraddau sylweddol uwch o dderbyniadau i'r ysbyty yn ystod eu blwyddyn gyntaf o fywyd. Wrth gael eu grwpio i lai na 5 mlwydd oed a mwy na 5 mlwydd oed, canfuom fod plant a aned yn y tymor cynnar yn y ddau grŵp yn dweud eu bod yn llawer mwy o wich ac yn fwy diweddar yn siglo er bod y gwahaniaethau yn fwy amlwg yn y grŵp iau. Parhaodd y gwahaniaethau hyn pan gynhwyswyd hanes teuluol atopi a chyflwyno trwy adrannau cesaraidd mewn modelau atchweliad logistaidd a ddefnyddiwyd.
