Cefais Gymrodoriaeth Ddoethurol NIHR Ymchwil Iechyd a Gofal Cymru yn 2019 i gwblhau PhD ar seicometrig, mesurau canlyniadau a adroddir gan gleifion (PROMs), theori mesur Rasch a phrawf addasol cyfrifiadurol ym maes Gweithgaredd Clefyd Arthritis Rhewmatoid. Cefais fy ngoruchwylio gan yr Athro Ernest Choy (Prifysgol Caerdydd), Dr Mike Horton (Prifysgol Leeds), Dr Karl Bang Christensen (Prifysgol Copenhagen), Dr Rhiannon Phillips (Prifysgol Metropolitan Caerdydd) a Dr David Gillespie (Prifysgol Caerdydd).
Cefais Wobr Camau Nesaf Ymchwil Iechyd a Gofal Cymru i barhau â'r ymchwil o'm PhD o'r enw astudiaeth PLAN-HERACLES.
Rwyf wedi dal swyddi yng Nghanolfan Trin a Gwerthuso Arthritis Arbrofol Rhanbarthol Caerdydd (CREATE) o'r blaen ac rwyf wedi cael secondiad yn yr Ysgol Deintyddiaeth, a oedd yn ogystal ag ymchwil, yn cynnwys addysgu ystadegau a SPSS i fyfyrwyr BDS, a darparu cymorth ystadegol i fyfyrwyr MOrth a PhD.
Hyd at 2019, roedd fy ngwaith yn CTR yn bennaf yn cynnwys bod yn ystadegydd ar dreialon clinigol lluosog a gwahanol, ar draws llawer o ddisgyblaethau amrywiol. Rwyf wedi datblygu diddordeb mewn diabetes, heintiau llwybr wrinol, rhiwmatoleg a chyflyrau croen, yn ogystal â caries deintyddol o fy ngwaith blaenorol yn yr Ysgol Deintyddiaeth. Yr ymyriadau rydw i wedi canolbwyntio arnynt yw cwnsela newid ymddygiad, offer gwneud penderfyniadau a rennir a dyfeisiau/cymhorthion ar gyfer presgripsiynu gwrthfiotigau priodol.
Yn fy rôl o fewn y ganolfan CREATE, roeddwn yn ymchwilio i'r defnydd o fethodoleg treialon clinigol cyfnod cynnar ym maes rhiwmatoleg. Roeddwn eisoes wedi dechrau edrych ar dechnegau i archwilio priodweddau seicometrig amrywiol Offerynnau Ansawdd Bywyd, ac roeddwn yn ffodus i dreulio fy nhraethawd hir MSc yn dysgu am, ac yn ymgymryd â dadansoddiadau Rasch, sy'n thema ymchwil barhaus.
Yn dilyn cais llwyddiannus, ymunais â charfan 2019 y GW4 Crucible ar Arloesedd Digidol a derbyniais gyllid hadau ar gyfer astudiaeth GW4-PATH.
Roeddwn i'n rhan o 2024/25 Cynnau | Rhaglen diwylliant ymchwil positif Ignite a ddatblygwyd gan Brifysgol Caerdydd.

Mae Arthritis Rhewmatoid (RA) yn arthritis llidiol cronig cyffredin sy'n cael ei nodweddu gan ddifrifoldeb symptomau amrywiol a fflamau ailadroddus, sy'n gofyn am fonitro parhaus ac addasiadau triniaeth dilynol dros amser. Mae monitro gweithgaredd clefydau (DA) yn safon gofal yn RA. Mae'r asesiadau DA cyfredol yn gofyn am brofion labordy a / neu fewnbwn gweithwyr gofal iechyd proffesiynol (HCP). Efallai y bydd Mesurau Canlyniadau a Adroddir gan Gleifion (PROMs), sef offer a gwblhawyd gan gleifion i ganfod canfyddiadau o'u hiechyd, felly yn well gennych. Fodd bynnag, nid oes consensws ar sut i fesur RA DA gan ddefnyddio PROM.
Mae monitro gweithgaredd clefydau (DA) yn safon gofal mewn Arthritis Rhewmatoid (RA). Mae'r asesiadau DA cyfredol yn gofyn am brofion labordy a / neu fewnbwn proffesiynol gofal iechyd. Felly, efallai y bydd Mesurau Canlyniadau a Adroddir gan Gleifion (PROMs) yn well gennych. Fodd bynnag, nid oes consensws ar sut i fesur RA DA gan ddefnyddio PROM.
Nod roeddwn i i asesu priodweddau mesur PROMs RA DA etifeddiaeth a PROMs perthnasol eraill, a dechrau datblygu RA DA PROM newydd.
Defnyddiais sawl dull, gan gynnwys adolygiad systematig yn dilyn canllawiau COSMIN, dadansoddiadau meintiol o ddata ar PROMs etifeddol a gasglwyd mewn astudiaeth drawsdoriadol o bobl sy'n byw gydag RA (plwRA) ar draws pedwar Bwrdd Iechyd Prifysgol De Cymru, dadansoddiadau thematig a chynnwys o gyfweliadau gwybyddol gyda plwRA a datblygu prawf addasol cyfrifiadurol.
I found that no legacy RA DA PROMs can be recommended for future use and no legacy PROMs could fully evidence fit to the Rasch measurement theory model. Dangosais fod y parth byd-eang Cleifion yn ddau faes gwahanol o weithgaredd clefydau ac iechyd cyffredinol. Gellir defnyddio 12 eitem ar draws y meysydd Poen, Gweithgaredd Clefyd, Tynerwch a chwyddo, Gweithrediad corfforol ac Anystwythder i fesur adeiladwaith RA DA. Fe wnes i sefydlu rhywfaint o dystiolaeth gychwynnol o ddilysrwydd cynnwys ar gyfer yr eitemau hyn. Yn olaf, darganfyddais nad yw prawf addasol cyfrifiadurol yn darparu mantais fawr at ddibenion gweinyddu'r 12 eitem.
Mae fy nghanfyddiadau'n dangos y gellir mesur adeiladwaith RA DA gyda dim ond pum eitem, gydag un o bob un o'r meysydd Poen, Gweithgaredd Clefyd, Tynerwch a chwyddo, Gweithrediad corfforol a Stiffness. Y camau nesaf yw darganfod sut orau i ddylunio'r pum eitem hyn ac i brofi eu priodweddau mesur, cyn eu defnyddio fel rhan o offeryn monitro DA wythnosol.
