Rwy'n ymarferydd cyffredinol academaidd ac ymchwilydd gwasanaethau iechyd sy'n arwain ymchwil i sut mae sefydliadau iechyd a gofal cymdeithasol yn dysgu o ofal anniogel a brofir gan gleifion a theuluoedd.
Rwy'n cynnull y Grŵp Ymchwil Diogelwch Cleifion (y 'grŵp PISA') yn yr Is-adran Meddygaeth Poblogaeth, Ysgol Meddygaeth, Prifysgol Caerdydd, ac mae ein portffolio o ymchwil yn cael ei gefnogi gan NIHR, y Sefydliad Iechyd, y Sefydliad hwn, Ymchwil Canser y DU, Ymchwil Iechyd a Gofal Cymru ac mae'n cynnwys:
- ymchwilio i natur a baich niwed y gellir ei osgoi mewn gofal iechyd;
- nodi meysydd blaenoriaeth diogelwch cleifion o ddadansoddi data diogelwch cleifion arferol (e.e. adolygiad o nodiadau achos, adroddiadau digwyddiadau);
- arloesedd methodolegol ar gyfer rhannu dysgu o ymchwilio i ddigwyddiadau diogelwch cleifion;
- mesur diogelwch cleifion (datblygu tacsonomeg); a
- datblygu a gweithredu ymyriadau i leihau niwed i gleifion mewn lleoliadau iechyd a gofal cymdeithasol.
Ym mis Ebrill 2023, ymunais â Chanolfan Ymchwil Marie Curie yn yr Is-adran Meddygaeth Boblogaeth fel cyd-arweinydd ar gyfer ymchwil Diogelwch Cleifion a Gwella Gofal Iechyd.
Ar draws Prifysgol Caerdydd, rwy'n cynnull Cynghrair Ergonomeg a Chleifion Mwy Diogel Cymru (WESPA), grŵp rhyngddisgyblaethol o ymchwilwyr (Ysgol Meddygaeth, Ysgol Busnes Caerdydd, Ysgol Peirianneg) a chlinigwyr sy'n cynnal ymchwil a gwerthuso gwasanaethau i alluogi arloesi a gweithredu arferion i wella diogelwch cleifion mewn gofal iechyd.

Fi yw'r Arweinydd Pecyn Gwaith Diogelwch Cleifion yng Nghanolfan Ymchwil Gofal Sylfaenol a Gofal Brys Cymru (Canolfan PRIME Cymru).
Yn ystod pandemig COVID-19, roeddwn yn aelod o Grŵp Iechyd Cyhoeddus Brys y DU (NIHR / DHSC / CMOs) a'r Panel Gweithredu Dealltwriaeth a Dileu Treialon COVID-19 dilynol (CUE-TIP), Grŵp Cyflenwi Ymchwil Brechlyn COVID-19 Cymru, a chyfrannu at frechlyn a darparu treialon therapiwtig.
Rwy'n Gynghorydd Gwyddonol i raglen GIG Addysg i'r Alban ar 'Ymchwil Diogelwch Cleifion a Gwella Ansawdd a Datblygu Addysgol' ac yn aelod o'r Grŵp Cyfeirio Gofal Sylfaenol ar gyfer rhaglen genomeg Iechyd Ein Dyfodol.

Rwyf wedi datblygu dull dulliau cymysg ar gyfer ymchwilio i amlder ac osgoi niwed sylweddol mewn gofal iechyd; a nodi meysydd blaenoriaeth diogelwch cleifion o ddadansoddi data diogelwch cleifion ar gyfer ysgogi gwybodaeth gyda rhanddeiliaid lluosog gan gynnwys sefydliadau'r GIG a llunwyr polisi.
Datblygwyd fy datblygiadau methodolegol ar gyfer cynhyrchu dysgu o ddigwyddiadau diogelwch cleifion yn ystod astudiaeth agenda genedlaethol o ddigwyddiadau diogelwch cleifion mewn Ymarfer Cyffredinol (wedi'i ariannu gan raglen Ymchwil Gwasanaethau Iechyd a Chyflenwi NIHR, ac a elwir ar lafar fel 'astudiaeth PISA').
Astudiaeth PISA oedd y nodwedd fwyaf o ddigwyddiadau diogelwch cleifion mewn practis cyffredinol ledled y byd. Mae allbynnau methodolegol astudiaeth PISA wedi darparu sylfaen i ymchwilwyr eraill efelychu ac ehangu'r ymchwil, a hyrwyddo'r agenda diogelwch cleifion gofal sylfaenol, yn rhyngwladol. Yn y DU, er enghraifft, fe'u cymhwyswyd i nodi 'niwed sylweddol y gellir ei osgoi' mewn Meddygfeydd Cyffredinol yn Lloegr (a ariennir gan Raglen Ymchwil Polisi NIHR) a'r astudiaeth a ariennir gan NIHR, i ymchwilio i niwed y gellir ei osgoi mewn gwasanaethau gofal iechyd carchardai yn Lloegr.
Nododd astudiaeth PISA a ariannwyd gan NIHR & DR, (2013-15) i nodweddu digwyddiadau diogelwch cleifion sy'n digwydd mewn practis cyffredinol amrywiaeth o grwpiau cleifion agored i niwed a gwendid systemig a oedd yn cyfiawnhau ymchwiliad manylach.
Yn dilyn hynny, mae'r grŵp PISA wedi arwain astudiaethau mawr o feysydd blaenoriaeth a nodwyd ar gyfer diogelwch cleifion ar draws y continwwm iechyd a gofal cymdeithasol, gan gynnwys: rhyddhau anniogel o leoliadau gofal eilaidd i ofal sylfaenol a gwallau a brofir gan blant mewn gofal sylfaenol, oedolion hŷn, cleifion sy'n derbyn gofal lliniarol , cynllunio gofal uwch, cleifion â dementia, oedolion sy'n derbyn gwasanaethau iechyd meddwl mewn gofal sylfaenol, ac oedolion sy'n derbyn opiate ailosod.

Dysgu sut i wella diogelwch cleifion mewn gwahanol gyd-destunau iechyd a gofal cymdeithasol (gan gynnwys cartrefi gofal)
Defnyddiwyd canfyddiadau o ddadansoddiadau blaenorol o ddigwyddiadau diogelwch cleifion i lywio dyluniad mentrau a phrosiectau gwella ansawdd yn empirig i wella diogelwch cleifion mewn sefydliadau gofal iechyd. Mae'r gwersi a ddysgwyd o'n hastudiaethau gofal sylfaenol yn cael eu defnyddio gan y gwasanaeth Gwella 1000 o Fywydau yng Nghymru i ddylunio eu strategaeth gwella ar lefel genedlaethol ar gyfer diogelwch cleifion gofal sylfaenol. Ar lefel leol, defnyddiodd un bwrdd iechyd yng Nghymru, ein dadansoddiad o adroddiadau am gamgymeriadau sy'n gysylltiedig â gwrthgeulo i dynnu sylw at risgiau i gleifion sy'n cael eu cychwyn ar Warfarin yn yr ysbyty. Arweiniodd y prosiect gwella ansawdd dilynol at Wasanaeth Gwell dan Gyfarwyddyd cenedlaethol ar gyfer darparu gwasanaethau gwrthgeulo i gleifion mewn lleoliadau cymunedol yn lle hynny (am fwy o fanylion, gwyliwch fideo byr ar YouTube).
Yn ogystal, rydym yn cysyniadu, ymchwilio a chefnogi timau i wella diogelwch cleifion ar gyfer grwpiau cleifion agored i niwed ar draws y continwwm gofal. Er enghraifft, gyda chymorth gan Goleg Brenhinol yr Ymarferwyr Cyffredinol a chymrodoriaeth a ariennir gan Marie Curie, rydym wedi datblygu model 'ymchwilydd-preswyl' o weithio mewn bwrdd iechyd mawr yng Nghymru i werthuso prosiect gwella ansawdd gyda'r nod o wella gofal diwedd oes yn y lleoliad gofal y tu allan i oriau meddyg teulu.
Rydym yn gwerthuso datblygiad, profi a gweithredu ymyriadau diogelwch cleifion i ddeall sut ac ym mha gyd-destunau y gallant wella canlyniadau. Mae'r Grŵp PISA yn defnyddio dull gwerthuso proses sy'n cael ei yrru gan theori a ddatblygwyd gyda chydweithwyr yn Ysgol Feddygol Harvard a'r Sefydliad Gwella Gofal Iechyd (gweler Parry, Carson-Stevens et al. 2013 am fwy o fanylion).
Adeiladu gallu a gallu i alluogi pontio o 'ddysgu i weithredu'
Rwy'n gweithio'n agos gyda sefydliadau'r GIG i weithredu arloesiadau methodolegol ac archwilio sut i gynyddu cyrhaeddiad ac effaith y buddion i gleifion a'u teuluoedd (gan gynnwys rhoddwyr gofal anffurfiol), er enghraifft, trwy:
Mae ymdrechion blaenorol i adnabod a dysgu o'r ffynonellau niwed pwysicaf i gleifion wedi cael eu cyfyngu gan ddiffyg system safonol gyffredinol ar gyfer dosbarthu difrifoldeb niwed ac esgeulustod cyffredinol niwed seicolegol yn y cyd-destun hwn. Mae fy ngrŵp ymchwil wedi datblygu cyfres o systemau dosbarthu yn empirig, er enghraifft y System Dosbarthu Difrifoldeb Niwed Gofal Sylfaenol a gyhoeddwyd ym Mwletin Sefydliad Iechyd y Byd, i'w defnyddio'n rhyngwladol, ar draws lleoliadau gofal sylfaenol, i wella canfod ac atal digwyddiadau sy'n achosi'r niwed mwyaf difrifol i gleifion.
Mae profiadau gofal dirdynnol, anniogel cleifion a'u teuluoedd yn cael eu darlunio mewn adroddiadau digwyddiadau diogelwch cleifion. Mae adroddiadau o'r fath yn cynrychioli persbectif unigryw ar gyfer dysgu. Fodd bynnag, mae maint y data mewn llawer o systemau adrodd am ddigwyddiadau diogelwch cleifion mor fawr fel nad oes llawer erioed wedi'i ddadansoddi na'i ddefnyddio i gefnogi gwelliant mewn diogelwch cleifion.
Rydym wedi datblygu dulliau dysgu peiriannau (hy dulliau dosbarthu testun) i oresgyn yr her hon a fydd yn awtomeiddio casglu gwybodaeth hanfodol i ddeall digwyddiadau diogelwch cleifion gan gynnwys tynnu manylion am yr hyn a ddigwyddodd (math o ddigwyddiad), pam y digwyddodd (ffactorau cyfrannol) a difrifoldeb y canlyniad (difrifoldeb niwed).
Mae sefydliadau hefyd wedi cael eu rhwystro gan ddiffyg buddsoddiad ar gyfer meithrin gallu a gallu staff i ddadansoddi data o'r fath. Gyda chefnogaeth Gwobr Hyrwyddo Analytics gan Sefydliad Iechyd, rydym ar hyn o bryd yn archwilio dulliau o 'harneisio dadansoddeg data i wneud y mwyaf o ddysgu'r GIG o adroddiadau digwyddiadau diogelwch cleifion' ac yn gweithio i wireddu'r synergedd rhwng dadansoddwyr data, rheolwyr a chlinigwyr ar gyfer nodi a gweithredu ar ddysgu o ddata diogelwch cleifion.
Mae'r gwersi a ddysgwyd o fy mhrosiectau ymchwil wedi cael eu lledaenu gyda chefnogaeth Coleg Brenhinol yr Ymarferwyr Cyffredinol i hyfforddi'r gweithlu i gydnabod, adrodd a dysgu o ddigwyddiadau diogelwch cleifion trwy gyrsiau e-ddysgu, seminarau cenedlaethol, a chanllaw ymarferol 'sut i '.
Mae Grŵp PISA yn ymdrechu i feithrin gallu a gallu ymchwilwyr y gwasanaeth iechyd i ymchwilio i ddiogelwch cleifion (myfyrwyr ôl-raddedig, academyddion clinigol, cymrodyr ôl-ddoethurol) trwy ymweld ag apwyntiadau gyda'r grŵp PISA .

Trosolwg o ysgolheictod addysgol
Yn rhyngwladol, rwyf wedi rhannu'r dulliau dull cymysg arloesol a ddatblygwyd gan fy ngrŵp ymchwil i ymchwilio a deall epidemioleg digwyddiadau diogelwch cleifion gydag ymchwilwyr eraill. Rwyf wedi hyfforddi dros 70 o ymchwilwyr gwasanaethau iechyd i ymchwilio i ddigwyddiadau diogelwch cleifion ac mae hyn wedi arwain at nifer o wahoddiadau rhyngwladol ar gyfer cynnal ymchwil a chyd-ysgrifennu cyhoeddiadau.
Roeddwn yn ddarlithydd gwadd yn Ysgol Iechyd y Cyhoedd Harvard Chan yn 2017 ac yn cyd-ddysgu cwrs wythnos ar ddulliau epidemiolegol ar gyfer deall diogelwch cleifion. Rwyf hefyd yn ymdrechu i gefnogi staff, rheolwyr ac arweinwyr gofal iechyd rheng flaen i ddysgu o ofal iechyd anniogel; er enghraifft, fi oedd yr Arweinydd Clinigol ar gyfer Gwella Ansawdd mewn Diogelwch Cleifion yng Ngholeg Brenhinol yr Ymarferwyr Cyffredinol (2016-17) ac fe wnes i gyd-ysgrifennu 'RCGP Guide for Reporting and Learning from Patient Safety Incidents in General Practice' a datblygais ddau fodiwl ar-lein a gynhaliwyd gan RCGP Learning. Rwyf hefyd wedi cyfrannu'n rhyngwladol at raglen OPEN Pediatrics Ysbyty Plant Boston / Ysgol Feddygol Harvard, wedi'i hanelu'n bennaf at ddysgwyr o leoliadau incwm isel a chanolig, ar ddatgelu ac ymddiheuro i gleifion a theuluoedd yn dilyn gofal iechyd anniogel.
Rhwng 2012-16, fi oedd Arweinydd Cyfadran Sefydliad Gwella Gofal Iechyd y DU ac Iwerddon ar gyfer y rhaglen addysgol ar-lein a chymunedol, Ysgol Agored yr IHI (2012-16) - bellach y darparwr mwyaf o addysg gwella ansawdd a diogelwch cleifion ledled y byd.
Yn 2008, roeddwn yn intern i'r Athro Donald Berwick yn y Sefydliad Gwella Gofal Iechyd yng Nghaergrawnt, UDA. Roeddwn i'n gyd-sylfaenydd Ysgol Agored yr IHI. Gan ddefnyddio dulliau trefnu cymdeithasol a rhwydwaith cynyddol Ysgol Agored yr IHI o selogion gwella ansawdd, roeddwn yn gyd-sylfaenydd ymgyrch diogelwch cleifion byd-eang ar gyfer gweithwyr gofal iechyd proffesiynol iau o'r enw "Check a Box." Achub bywyd." cefnogi lledaeniad a lledaeniad Rhestr Wirio Diogelwch Llawfeddygol WHO . Yn y blynyddoedd diweddarach, fel aelod o'r gyfadran, datblygais ddulliau i fyfyrwyr ddysgu am brofiad cleifion a theuluoedd mewn gofal iechyd i lywio gwelliant ansawdd drwy ofyn Un Cwestiwn – annog myfyrwyr i fabwysiadu strategaethau syml fel gofyn i bob claf y maent yn ei gyfarfod, "Beth alla i ei wneud i wella'ch gofal heddiw?". Ers hynny, mae'r datblygiadau arloesol hyn wedi cael eu rhoi ar waith mewn sawl cwricwla meddygol, er enghraifft, ym Mhrifysgol Caerdydd a Phrifysgol British Columbia (Canada).
