Ym Mhrifysgol Caerdydd, rwy'n ymgynnull y Grŵp Ymchwil Diogelwch Cleifion (y 'grŵp PISA') gydag arbenigedd sy'n ymchwilio i natur a baich niwed y gellir ei osgoi mewn gofal iechyd, gan gynnwys datblygiadau methodolegol i fesur canlyniadau diogelwch ar draws cyd-destunau iechyd a gofal cymdeithasol. Mae gen i ddiddordeb arbennig mewn gweithio a gwireddu synergeddau rhwng disgyblaethau, ac ar draws sawl ysgol yn y brifysgol rwy'n cydlynu Cynghrair Ergonomeg a Chleifion Mwy Diogel Cymru (WESPA) sy'n grŵp rhyngddisgyblaethol o ymchwilwyr ac academyddion clinigol sy'n gweithio i wella diogelwch cleifion yn y GIG.

Dysgu o ganlyniadau iechyd a gofal cymdeithasol anniogel a brofwyd gan gleifion a'u teuluoedd
Rwyf wedi datblygu dull dulliau cymysg ar gyfer mesur amlder ac osgoi niwed, a nodi blaenoriaethau ar gyfer gwella diogelwch.
Mae'r system ddosbarthu Diogelwch Cleifion (PISA), sy'n cynnwys fframweithiau codio aml-echelinol, wedi'i halinio â Dosbarthiad Rhyngwladol WHO ar gyfer Diogelwch Cleifion, ac fe'i datblygwyd yn empirig yn ystod astudiaeth genedlaethol sy'n gosod agenda o ddigwyddiadau diogelwch cleifion mewn Practis Gyffredinol (a ariennir gan raglen Ymchwil Gwasanaethau Iechyd a Chyflenwi NIHR ac a elwir ar lafar yn 'astudiaeth PISA').
Astudiaeth PISA oedd y nodweddiad mwyaf o ddigwyddiadau diogelwch cleifion mewn practis cyffredinol ledled y byd. Ers hynny, mae'r system ddosbarthu PISA wedi darparu sylfaen i ymchwilwyr eraill ailadrodd ac ymestyn yr ymchwil, a hyrwyddo'r agenda diogelwch cleifion gofal sylfaenol, yn rhyngwladol. Yn y DU, er enghraifft, mae PISA wedi cael ei gymhwyso i nodi 'niwed sylweddol y gellir ei osgoi' mewn Arferion Cyffredinol yn Lloegr (a ariennir gan Raglen Ymchwil Polisi NIHR ), yr astudiaeth a ariennir gan NIHR i ymchwilio i niwed y gellir ei osgoi mewn gwasanaethau gofal iechyd carchardai yn Lloegr, a Think Cancer! Treial yn archwilio achosion yr oedi hiraf ar gyfer diagnosis canser.
Mae gan y grŵp PISA bortffolio helaeth o astudiaethau wedi'u cwblhau sydd wedi nodi blaenoriaethau gwella diogelwch ar draws y continwwm iechyd a gofal cymdeithasol, gan gynnwys: rhyddhau anniogel o leoliadau gofal eilaidd i leoliadau gofal sylfaenol a digwyddiadau diogelwch a brofwyd gan blant mewn gofal sylfaenol, oedolion hŷn, cleifion sy'n derbyn gofal lliniarol, cynllunio gofal uwch, cleifion â dementia, oedolion sy'n derbyn gwasanaethau iechyd meddwl mewn gofal sylfaenol, ac oedolion sy'n derbyn amnewid opiate.

Adeiladu gallu a gallu i alluogi pontio o 'ddysgu i weithredu'
Ledled y byd, mae ymdrechion i adnabod a dysgu o'r ffynonellau pwysicaf o niwed i gleifion wedi cael eu cyfyngu gan ddiffyg system safonol gyffredinol ar gyfer dosbarthu difrifoldeb niwed a chreulustod cyffredinol niwed seicolegol yn y cyd-destun hwn. Mae fy ngrŵp ymchwil wedi datblygu cyfres o systemau dosbarthu, er enghraifft System Dosbarthu Difrifoldeb Niwed PISA a gyhoeddwyd ym Mwletin Sefydliad Iechyd y Byd, i'w cymhwyso'n rhyngwladol i wella canfod ac atal digwyddiadau sy'n achosi'r niwed mwyaf difrifol i gleifion.

Mae profiadau gofal anniogel cleifion a'u teuluoedd yn rhy aml yn cael eu darlunio mewn adroddiadau digwyddiadau diogelwch cleifion. Mae adroddiadau o'r fath yn cynrychioli persbectif unigryw ar gyfer dysgu. Fodd bynnag, mae cyfaint y data mewn llawer o systemau adrodd digwyddiadau diogelwch cleifion mor fawr fel nad yw llawer erioed wedi cael eu dadansoddi na'u defnyddio i gefnogi gwelliant mewn diogelwch cleifion. Rydym wedi datblygu dulliau dysgu peirianyddol (h.y. dulliau dosbarthu testun) i oresgyn yr her hon a fydd yn awtomeiddio cipio gwybodaeth hanfodol i ddeall digwyddiadau diogelwch cleifion gan gynnwys tynnu manylion am yr hyn a ddigwyddodd (math o ddigwyddiad), pam y digwyddodd (ffactorau cyfrannu) a difrifoldeb y canlyniad (difrifoldeb niwed).

Trosolwg o ysgolheictod addysgol
Yn rhyngwladol, rwyf wedi rhannu gydag ymchwilwyr eraill y dulliau dull cymysg arloesol a ddatblygwyd gan fy ngrŵp ymchwil i ymchwilio a deall epidemioleg digwyddiadau diogelwch cleifion (darlithydd gwadd yn Ysgol Iechyd Cyhoeddus Harvard Chan yn 2017). Rwyf wedi hyfforddi ymchwilwyr i ddysgu o ddigwyddiadau diogelwch cleifion gan ddefnyddio'r Dull PISA yn genedlaethol ac yn fyd-eang (Awstralia, Brasil, Canada, Ffrainc, Kuwait, Mecsico, Sbaen, Twrci, UDA) gan arwain at lawer o gydweithrediadau ymchwil rhyngwladol ac allbynnau effeithiol. Mae'r Grŵp PISA yn croesawu mynegiadau o ddiddordebau gan y rhai sy'n ceisio hyrwyddo eu hyfedredd ymchwil diogelwch cleifion (myfyrwyr ôl-raddedig, academyddion clinigol, cymrodyr ôl-ddoethurol, uwch academyddion) drwy apwyntiadau ymweld ym Mhrifysgol Caerdydd.
Mae gwersi a ddysgwyd o fy astudiaethau ymchwil wedi dylanwadu ar strategaeth Sefydliad Iechyd y Byd ar gyfer diogelwch cleifion. Mae dysgu yn cael ei rannu'n ôl i ddarparwyr rheng flaen trwy systemau addysgol sefydledig fel Coleg Brenhinol yr Ymarferwyr Cyffredinol; mae un enghraifft yn cynnwys rhaglen sydd wedi'i chynllunio i gefnogi'r gweithlu gofal sylfaenol i gydnabod, adrodd a dysgu o ddigwyddiadau diogelwch cleifion drwy gyrsiau e-ddysgu, seminarau cenedlaethol, a chanllaw ymarferol 'sut i'. Rwyf hefyd wedi cyfrannu'n rhyngwladol at raglen OPEN Pediatrics Ysbyty Plant Boston / Ysgol Feddygol Harvard, sydd wedi'i anelu'n bennaf at ddysgwyr o leoliadau incwm isel a chanolig, ar ddatgelu ac ymddiheuriad i gleifion a theuluoedd yn dilyn gofal iechyd anniogel.
O 2012-16, roeddwn yn Arweinydd Cyfadran Sefydliad Gwella Gofal Iechyd y DU ac Iwerddon ar gyfer y rhaglen addysgol ar-lein a chymunedol, Ysgol Agored IHI (2012-16) – bellach y darparwr mwyaf o addysg gwella ansawdd a diogelwch cleifion ledled y byd. Yn 2008, roeddwn yn intern i'r Athro Donald Berwick yn y Sefydliad Gwella Gofal Iechyd yng Nghaergrawnt, UDA. Roeddwn i'n un o gyd-sylfaenwyr Ysgol Agored IHI. Gan ddefnyddio dulliau trefnu cymdeithasol a rhwydwaith cynyddol Ysgol Agored IHI o selogion gwella ansawdd, roeddwn yn gyd-sylfaenydd ymgyrch diogelwch cleifion byd-eang ar gyfer gweithwyr gofal iechyd proffesiynol iau o'r enw "Check a Box. Achub Bywyd." gan gefnogi lledaeniad a lledaenu Rhestr Wirio Diogelwch Llawfeddygol WHO . Yn y blynyddoedd diweddarach, fel aelod o'r gyfadran, datblygais ddulliau i fyfyrwyr ddysgu am brofiad cleifion a theuluoedd mewn gofal iechyd i lywio gwella ansawdd trwy Gofyn Un Cwestiwn – annog myfyrwyr i fabwysiadu strategaethau syml fel gofyn i bob claf maen nhw'n cwrdd, "Beth alla i ei wneud i wella eich gofal heddiw?". Ers hynny, mae'r arloesiadau addysgol hyn wedi'u gweithredu mewn sawl cwricwlwm meddygol, er enghraifft, ym Mhrifysgol Caerdydd a Phrifysgol British Columbia (Canada).
