Mae gen i ddiddordeb ymchwil clinigol a throsiadol mewn sawl agwedd ar endocrinoleg, gyda ffocws penodol ar endocrinoleg atgenhedlu, clefyd y thyroid ac anhwylderau steroid.
Mae fy astudiaethau presennol yn defnyddio cofnodion iechyd electronig, dadansoddi genetig ac MRI uwch i ddeall effaith yr amgylchedd hormonaidd mewn bywyd cynnar ar ganlyniadau iechyd tymor hir. Mae fy ngrŵp wedi nodweddu canlyniadau metabolig, iechyd meddwl a cardiofasgwlaidd hirdymor cleifion â Syndrom Ofari Polysystig (PCOS), ac mae'n archwilio effaith PCOS ar ddefnyddio adnoddau iechyd a baich economaidd. Rydym wedi cynnal astudiaethau ffisiolegol i ddangos newid microstructure, metaboledd a swyddogaeth serebrofasgwlaidd yn y boblogaeth hon, ac rydym yn archwilio'r potensial ar gyfer therapi ymarfer corff a blocâd androgen i addasu'r risgiau hyn. Ar hyn o bryd, rwy'n arwain Partneriaeth Blaenoriaethu Cynghrair James Lind yn PCOS.
Mae ein hastudiaeth ddilynol hirdymor o blant a anwyd i famau yn y treial Sgrinio Thyroid Antenatal Rheoledig (CATS) yn cynnwys MRI uwch a gynhaliwyd yng Nghanolfan Ymchwil Delweddu'r Ymennydd Prifysgol Caerdydd (CUBRIC) i archwilio effaith statws thyroid mamol diwygiedig ar niwroddatblygiad. Rydym wedi defnyddio Banc Data SAIL i ddangos mwy o risg o ddigwyddiadau cardiofasgwlaidd mewn cleifion sydd â Gwrthwynebiad i Hormon Thyroid-Beta, gyda'r bwriad o archwilio therapïau newydd a allai liniaru'r risg hon.
Mae ein gwaith mewn anhwylderau steroid wedi dangos gwerth cortisol poer fel dewis arall i fesur serwm ar gyfer diagnosis annigonolrwydd adrenal. Bydd ein hastudiaeth ddilynol hirdymor o blant a anwyd i famau yn y treial Sgrinio Thyroid Antenatal Rheoledig (CATS) yn ein galluogi i asesu effeithiau amlygiad hormonau thyroid yn ystod beichiogrwydd ar niwroddatblygiad, gan ddefnyddio'r cyfleusterau MRI diweddaraf yng Nghanolfan Ymchwil Delweddu'r Ymennydd Prifysgol Caerdydd (CUBRIC).
Mae dadansoddiadau epidemiolegol a genetig mewn setiau data mawr, gan gynnwys CPRD a Banc Bio'r DU, yn archwilio morbidrwydd cardiofasgwlaidd mewn menywod â PCOS, ac effeithiau statws thyroid mamol ar strwythur yr ymennydd a chanlyniadau addysgol. Mae astudiaethau mewn ymwrthedd i hormon thyroid beta yn cynnwys astudiaeth banc data a fydd yn amlinellu canlyniadau iechyd tymor hir a threialon clinigol therapïau newydd.
Bydd Partneriaeth Gosod Blaenoriaeth Cynghrair James Lind yn PCOS yn sefydlu'r 10 blaenoriaeth ymchwil orau ar gyfer yr anhwylder hwn.

Ardrawiad
Rwyf wedi sefydlu carfan fawr o gleifion ifanc â Syndrom Ofari Polysystig (PCOS), gan ddefnyddio hyn fel model clinigol cyffredin (5-10% o fenywod cynmenopawsol) o ordewdra metabolicaly afiach a risg fasgwlaidd bywyd cynnar.
Defnyddiais gyfres o fethodolegau cardiofasgwlaidd anfewnwthiol i ffenoteipio'r garfan yn ofalus a dangos pwysigrwydd ymwrthedd inswlin a gordewdra canolog fel marcwyr risg cardiofasgwlaidd annibynnol. Cynhaliodd fy ngrŵp dreial ar hap, dwbl, dall a reolir gan placebo i ddangos bod metformin yn gwella swyddogaeth fasgwlaidd mewn menywod â PCOS. Rwyf hefyd wedi manteisio ar bŵer y Gronfa Ddata Ymchwil Ymarfer Clinigol i ddiffinio hanes naturiol PCOS, gan gadarnhau risg uwch o ddiabetes math 2, canlyniadau niweidiol beichiogrwydd, canlyniadau iechyd meddwl niweidiol a digwyddiadau cardiofasgwlaidd, yn ogystal ag effaith sylweddol ar economi gofal iechyd y DU.
Mae gen i ddiddordeb cryf hefyd mewn anhwylderau steroid; mewn clefyd adrenal, arweiniais ddau dreial clinigol a ail-ddiffinio terfynau cyfeirio dull-benodol i'r prawf synacthen , dangos dilysrwydd ymatebion poeri ac archwilio effeithiau DHEA ar swyddogaeth fasgwlaidd. Rwyf wedi ymgymryd â nifer o astudiaethau mewn Hyperplasia Adrenal Cynhenid sydd wedi helpu i ddiffinio'r canlyniadau iechyd ac wedi cyfrannu at ddatblygu therapïau newydd.
Mae dilyniant i'r garfan astudio Sgrinio Thyroid Antenatal Rheoledig (CATS) wedi egluro pwysigrwydd osgoi disodli levothyroxine yn ystod beichiogrwydd, tra hefyd yn dangos effaith ar ficrostrwythur ymennydd glasoed.
Rwy'n lledaenu fy ngwaith yn rheolaidd drwy ymgysylltu â grwpiau cleifion, y Gymdeithas Endocrinoleg a'r cyfryngau (BBC, S4C, Radio Cymru).
Addysgu
Rwy'n cyfrannu at gyflwyno, dylunio ac asesu'r cwricwlwm meddygol israddedig o flynyddoedd 1 hyd at 5, ac yn chwarae rhan weithredol mewn addysgu meddygol ôl-raddedig ar lefelau lleol a chenedlaethol.
Is-raddedig
Rwy'n darparu addysgu cleifion allanol a ward i fyfyrwyr blwyddyn 3-5, ac yn goruchwylio prosiectau cydrannau a ddewiswyd gan fyfyrwyr ym mlynyddoedd 3 a 4. Ar ôl cyflwyno'r cwricwlwm israddedig C21 yng Nghaerdydd, cwblheais y Rhaglen Cyfeiriadedd Addysg Feddygol (MEOP), daeth yn diwtor ar 'hormonau atgenhedlu' ar gyfer y Llwyfan mewn Gwyddoniaeth Glinigol yng ngham 1a (blwyddyn 1) ac arweinydd ysgrifennu ar sail achos ('Diabetes mewn person hŷn') ar gyfer cam 1b (blwyddyn 2). Rwy'n fentor academaidd ar gyfer israddedigion meddygol ac wedi bod yn arholwr mewn arholiadau meddygol am y 15 mlynedd diwethaf. Rwy'n croesawu myfyrwyr yn rheolaidd ar raglen gyfnewid Erasmus. Dyfarnwyd Tystysgrif Rhagoriaeth Addysgu i mi gan Fwrdd Iechyd Prifysgol Caerdydd a'r Fro yn 2010. Rwy'n cyflwyno darlith flynyddol i israddedigion deintyddol ar anhwylderau steroid.
Ar lefel y DU, cynorthwyais i ddatblygu set o Safonau Cenedlaethol, a fabwysiadwyd ar y cyd gan y Gymdeithas Endocrinoleg, Cymdeithas Diabetolegwyr Clinigol Prydain (ABCD) a Diabetes UK, ar gyfer addysg israddedig mewn Endocrinoleg a Diabetes, wedi'i meincnodi yn erbyn Meddygon Yfory (2012). Yn 2015, golygais ac awdur Clinical Endocrinology and Diabetes at a Glance, cyfres testunau israddedig flaenllaw yn y DU mewn meddygaeth glinigol.
Ôl-raddedig
Fel arweinydd academaidd y Pwyllgor Hyfforddi Arbenigol ar gyfer Endocrinoleg a Diabetes yng Nghymru, sefydlais raglen addysgu niwroendocrin ar gyfer cofrestryddion arbenigol. Gyda chefnogaeth grant gan y Gymdeithas Endocrinoleg, cyflwynais sesiwn cyflwyno poster a datblygu gwobrau israddedig ac ôl-raddedig yng nghyfarfod blynyddol Cymdeithas Endocrin a Diabetes Cymru (WEDS). Yn dilyn hynny, fel cadeirydd WEDS, trefnais gyfarfod cyntaf y Gymdeithas Achosion Clinigol Rhanbarthol Endocrinoleg i'w gynnal yng Nghymru. Cefais fy newis fel Cynullydd ar gyfer cwrs Diweddariad Clinigol y Gymdeithas Endocrinoleg (2012-2018), lle gwnes i ddylunio a chyflwyno addysgu ar diwmorau niwroendocrin, neoplasia endocrin etifeddol, hyponatraemia a hypoglycaemia i dros 200 o Gofrestryddion Arbenigol ac Ymgynghorwyr bob blwyddyn. Roeddwn yn aelod o fwrdd Arholi Tystysgrif Arbenigedd Brenhinol y Meddygon ar gyfer Endocrinoleg a Diabetes (2020-2023) ac ar hyn o bryd rwy'n cyd-arwain ar gyfer adrannau Pituitary ac Adrenal rhaglen ESAP flynyddol Cymdeithas Endocrinaidd America.
