Mae gen i swyddi Microbiolegydd Ymgynghorol Anrhydeddus hefyd gydag Asiantaeth Diogelwch Iechyd y DU ac Ymddiriedolaeth GIG Cwm Taf Morgannwg ac Athro Cyswllt Atodol gyda Phrifysgol Gorllewin Awstralia ers 2014. Rwy'n awdurdod rhyngwladol blaenllaw ar spp Ureaplasma clinigol. a heintiau mycoplasma hominis a dal swydd Ysgrifennydd Cyffredinol ar gyfer Sefydliad Rhyngwladol Mycoplasmoleg o 2014-2021. Ar hyn o bryd mae fy labordy yn gwasanaethu fel Labordy Cyfeirio Profi Sensitifrwydd Gwrthficrobaidd y DU ar gyfer pob spp Ureaplasma. a M. hominis samplau clinigol cadarnhaol a gyflwynwyd i UKHSA (Iechyd Cyhoeddus Lloegr gynt) a gwasanaethau GIG Iechyd Cyhoeddus Cymru. Mae diweddariadau ar gyfer Microbioleg Feddygol yn cael eu postio ar gyfer grŵp Ymchwil Gwrthficrobaidd Cymru Gyfan (@AWARRe Twitter).
Ar hyn o bryd, mae Labordy Spiller yn arwain ymchwiliadau cydweithredol (a ariennir gan Sefydliad Ineos Rhydychen) o'r holl heintiau bacteriol Gram-positif mewn sepsis newyddenedigol sy'n tarddu o wledydd incwm isel i ganolig (LMIC) fel rhan o gam II BARNARDS (Baich Ymwrthedd Gwrthfiotig mewn Newydd-anedig o Gymdeithasau sy'n Datblygu). Susceptibility gwrthficrobaidd, dadansoddiad dilyniant genom cyfan, virulence a throsglwyddo genynnau ymwrthedd gwrthficrobaidd o un bacteria i'r llall yn cael eu a nodweddir ar gyfer llif gwaed ynysig bacteria Gram-positif. Ar hyn o bryd mae safleoedd ysbytai cydweithredol sy'n cymryd rhan wedi'u lleoli ym Mhacistan, Nigeria, Bangladesh, Rwanda, Ethiopia, De Affrica (cam 1 BARNARD), Niger (Combacte-Care), yr Aifft, Sierra Leone, Mozambique, Burundi (gobeithio fel ehangu fel rhan o gam 2 BARNARD) ac Uganda (CYNNYDD).
Mae astudiaethau eraill sy'n rhedeg ochr yn ochr yn ymchwilio i fecanweithiau sylfaenol ymwrthedd gwrthficrobaidd (AMR) a pathogenicity mewn heintiau clinigol sy'n cynnwys Mollicutes (bacteria heb wal gell) bacteria gram negatif (gan gynnwys Legionella pneumophila, Klebsiella pneumoniae ac E. coli) a heintiau bacteriol ymledol Gram-positif y DU mewn oedolion (gan gynnwys Streptococcus agalactiae, Streptococcus sinensis, Streptococcus viridans a Streptococcus cristatus). Mae'r labordy hefyd yn cynnwys cyfleuster dilyniannu genomau cyfan Bacteriol Prifysgol Caerdydd, sy'n cynnal prosiectau cydweithredol gyda Pharc Genynnau Cymru, Iechyd Cyhoeddus Cymru (prosiect ARGENT) a Sefydliad Ineos Rhydychen. Y grwpiau o ddiddordeb yw cleifion sepsis (newydd-anedig ac oedolion; ac wedi'u lleoli yn y DU, Cuba, Uganda a Ghana), achosion o legionellosis y DU, yn ogystal â gwneud diagnosis a thriniaeth o wrethritis/cervicitis amhenodol mewn cleifion iechyd rhywiol yng Nghymru. Mae ffoci ymchwil yn cynnwys dilysu profion diagnostig masnachol newydd, datblygu asiantau gwrthficrobaidd newydd, datblygu llwyfannau sgrinio AST trwybwn uchel ac archwilio genynnau pathogenig / ffyrnigrwydd a amheuir. Mae'r Labordy Spiller yn arbennig o falch o fod yr unig labordy o hyd sydd wedi newid parvum Ureaplasma yn enetig hyd yma i ganiatáu delweddu ex vivo ar gyfer astudiaethau heintiau esgynnol arbrofol (mewn cydweithrediad â Phrifysgol Caeredin). Oherwydd ehangu cydweithrediadau o fewn AWARRe mae Labordy Spiller hefyd yn chwarae rhan weithredol mewn ymchwil gydweithredol ar enynnau cyfryngu AMR sy'n dod i'r amlwg (megis mcr-1, NDM-5, ac ati) in vitro ac in vivo.
Yn 2021, mae Labordy Spiller hefyd wedi sefydlu safleoedd ymchwil dwyochrog gweithredol i ymchwilio i fynychder ymwrthedd gwrthficrobaidd yn Ysbyty Addysgu Cape Coast yn Ghana a'r Adran Obstetreg a Phrifysgol Makerere Gynaecoleg ac Ysbyty Cyfeirio Cenedlaethol Kawempe, Uganda.

Ymchwilio i sepsis newyddenedigol mewn gwledydd incwm isel i ganolig (LMIC)
Labordy Spiller yw'r canolbwynt ar gyfer nodweddu pathogenau Gram-positif sy'n cael eu hynysu o lif gwaed babanod newydd-anedig yn ystod 30 diwrnod cyntaf bywyd. Mae tueddiad gwrthficrobaidd, dadansoddiad dilyniant genom cyfan, ffyrnigrwydd a throsglwyddo genynnau ymwrthedd gwrthficrobaidd o un bacteria i'r llall yn cael eu nodweddu ar gyfer ynysu o Bacistan, Nigeria, Bangladesh, Rwanda, Ethiopia, De Affrica (cam 1 BARNARD), Niger (Combacte-Care), yr Aifft, Sierra Leone, Mozambique, Burundi (gobeithio yn ehangu fel rhan o gam 2 BARNARD) ac Uganda (CYNNYDD). Mae cydweithrediadau gweithredol ar gyfer yr astudiaethau hyn yn cynnwys Prifysgol Rhydychen, Médecins Sans Frontières a St. George's, Prifysgol Llundain. Mewn cydweithrediad ag Asiantaeth Diogelwch Iechyd y DU, mae labordy Spiller wedi patentio cyfrwng Legionella solet newydd (Legionella Antimicrobial Susceptibility And Resistance Universal Screening medium: LASARUS) sy'n disodli cyfrwng activated sy'n cynnwys siarcol. Mae'r cyfrwng newydd yn dryloyw ac nid yw'n amsugno gwrthficrobau fel pob agar solid traddodiadol - ac ar hyn o bryd mae'n cael ei addasu i gynnwys Legionella spp. cromogenau penodol. Bydd hyn yn galluogi awtomeiddio sgrinio Legionella - datblygiad allweddol i ymateb i'r profion cynyddol sy'n ofynnol gan ddeddfwriaeth Cyfarwyddeb Dŵr Yfed yr UE. Mae labordy Spiller hefyd ar hyn o bryd yn arwain safoni profion tueddiad gwrthficrobaidd rhyngwladol ar gyfer niwmonia Legionella a Legionella clinigol eraill. rhywogaethau mewn labordai cyfeirio Legionella ar draws Ewrop ac yn y CDC. Ymchwil Grŵp B Strep (Streptococcus agalactiae)
Mae gan y labordy ddiddordeb arbennig mewn nodweddu symudedd Elfennau Cydgysylltiol Integraidd sy'n cario crynhoadau o enynnau ymwrthedd gwrthfiotig ym Mhrydain. Y tu hwnt i'r gwaith sy'n archwilio sepsis newyddenedigol mewn LMICs, mae gan y labordy hefyd gydweithrediadau gweithredol gydag Asiantaeth Diogelwch Iechyd y DU, Sanger Sequencing Institute Caergrawnt a Phrifysgol St George's Llundain i nodweddu cynnydd mewn ymwrthedd gwrthficrobaidd mewn GBS ymledol ym mhob grŵp oedran.
Ymchwil Cleifion Iechyd Rhywiol
Labordy Spiller fu'r labordy cyfeirio yn y DU ar gyfer profi tueddiad gwrthficrobaidd o Ureaplasmas a Mycoplasmas ar gyfer Asiantaeth Diogelwch Iechyd y DU am y degawd diwethaf. Mae hyn yn deillio o arbenigedd a gydnabyddir yn rhyngwladol ar gyfer yr heintiau a drosglwyddir yn rhywiol. Mae tair ysgoloriaeth a ariannwyd o'r blaen wedi dod i gasgliad yn archwilio'r epidemioleg, amlder ymwrthedd gwrthficrobaidd a pathogenicity cleifion Iechyd Rhywiol Cymru mewn cydweithrediad â'n harweinydd clinigol Dr Lucy Jones. Mae'r astudiaethau hyn yn cynnwys: (1) Astudiaeth MYCO WELL D-ONE (2016-2017); IRAS ) yn archwilio 1000 o gleifion o Gymru sy'n mynychu clinigau GIG Iechyd Rhywiol cerdded i mewn yn Rhondda Cynon Taf ar gyfer heintiau gan parvum wreaplasma, wreaplasicum, hominis Mycoplasma a genitaliwm Mycoplasma. (2) Astudiaeth UROGEN (2017-2018; IRAS), a oedd yn cynnwys dilysiad cyntaf yr assay Mycoplasma IST3 (BioMerieiux, Ffrainc) ar 500 o gleifion iechyd rhywiol symptomatig sy'n mynychu clinigau iechyd rhywiol cerdded i mewn ym mharc Iechyd Keir Hardie ac Ysbyty Dewi Sant. Ehangodd y prosiect hwn i gynnwys profion diagnostig diwylliant cromogenig ar gyfer Neisseria spp., E. coli, Grŵp B Streptococcus, Candida, Trichomonas vaginalis, Gardnerella vaginalis, Staphylococcus a Enterococcus spp. a (3) Astudiaeth y Gwarcheidwad gwrthfiotig (2019-2022), mewn cydweithrediad ag Iechyd Cyhoeddus Cymru ac Ymddiriedolaeth Iechyd Prifysgol Cwm Taf, a archwiliodd ddichonoldeb cyfarwyddo therapiwtig wedi'i dargedu'n gyflym ymyrraeth gan ddefnyddio profion SpeeDx masnachol ar gyfer canfod heintiau sy'n gwrthsefyll Mycoplasma a Neisseria gonorrhoea.
Ar hyn o bryd mae ein labordy yn cydweithio ag Iechyd Cyhoeddus Cymru i ailwerthuso profion tueddiad gwrthficrobaidd seiliedig ar ddiwylliant ar gyfer gonorrhoea Neisseria hefyd (myfyriwr Meistr Leanne Davies).
Bywgraffiad
Microbiolegydd Canada o Vancouver yn wreiddiol, des i i'r DU i ddechrau i archwilio osgoi microbau imiwnedd cynhenid. Rhwng 2000 a 2005 sefydlais grŵp ymchwil annibynnol drwy Raglen Cymrodoriaeth Datblygu Gyrfa Ymddiriedolaeth Wellcome, cyn ymuno â'r Adran Iechyd Plant yn Hosbis Prifysgol Cymru yn 2005 i ddatblygu thema ymchwil newydd sy'n archwilio ymwrthedd gwrthficrobaidd mewn heintiau bacteriol babanod newydd-anedig cynamserol. Arweiniodd hyn at sefydlu labordy cyfeirio blaenllaw y DU ar gyfer ymchwiliadau i Mycoplasma ac Ureaplasma Heintiau. Mae'r bacteria di-wal celloedd hyn yn ei hanfod yn gwrthsefyll y rhan fwyaf o wrthficrobau a nhw yw prif achos genedigaeth cyn y tymor ac maent yn dod i'r amlwg fel pathogenau anadlol. Yn ddiweddar, cwblheais fy nhrydydd tymor etholedig fel Ysgrifennydd Cyffredinol Sefydliad Rhyngwladol Mycoplasmology.
Yn 2012, dechreuais gydweithrediad a ariannwyd gan y Gymdeithas Frenhinol â Phrifysgol Gorllewin Awstralia i ymchwilio i batholeg microbaidd mewnwythiennol a datblygu gwrthfiotigau newydd a oedd yn ddiogel ac yn effeithiol i'w defnyddio mewn menywod beichiog. Arweiniodd y cydweithrediadau hyn at dderbyn Athrawes Cynorthwyol atodol yn Ysgol Iechyd Menywod a Babanod Prifysgol Gorllewin Awstralia, Perth, Awstralia yn 2014. Arweiniodd llwyddiant y cydweithio rhwng Caerdydd ac Awstralia at ddyrchafiad i Athro Cyswllt yn 2021.
Hefyd yn 2015, ymunais ag uned Ymchwil Gwrthficrobaidd Cymru Gyfan, dan arweiniad yr Athro Timothy Walsh, ac rwyf wedi bod yn ehangu fy ymchwil o ymwrthedd gwrthficrobaidd i facteria eraill. O ganlyniad i 2017, mae gen i brosiectau ymchwil newydd sy'n nodweddu straen Streptococcus Grŵp B wedi'u hynysu o gleifion sepsis y DU a nodweddu ynysu Legionella pnuemophilia rhag achosion o glefyd y llengfilwyr, Fe'i gelwir hefyd yn legionellosis.
Yn 2020, cefais fy mhenodi'n Bennaeth Microbioleg Feddygol lle rwy'n parhau i gefnogi ac ehangu ein tîm o ymchwilwyr gan gynnwys ymchwilwyr ar ddechrau eu gyrfa a chlinigwyr academaidd. Ar hyn o bryd rwy'n arwain nodweddu ynysion llif gwaed Gram-positif mewn babanod newydd-anedig ledled Affrica a De-ddwyrain Asia mewn cydweithrediad â Sefydliad Ineos Rhydychen (24 safle ar draws 8 gwlad: BARNARDS cam 2), yn ogystal ag arwain y safoni rhyngwladol i uno'r Profion Ymwrthedd Gwrthficrobaidd ar gyfer Legionella ledled Ewrop.
