Trosolwg ymchwil
Mae fy niddordebau yn cynnwys ymchwil glinigol ac iechyd cyhoeddus. Mae'r prosiectau presennol yn cynnwys yr ymchwiliad i ddarparu gwasanaethau iechyd a chymdeithasol i unigolion â nam ar eu golwg, gwerthuso ardystio nam ar y golwg a gynhaliwyd gan optometryddion arbenigol golwg isel, diogelwch cleifion yn ymwneud â gofal llygaid a'r gefnogaeth i optometryddion rhagnodol annibynnol sydd newydd gymhwyso. Mae prosiectau diweddar wedi archwilio canlyniadau adfer golwg. Mae meysydd penodol eraill yn cynnwys mesurau o swyddogaeth weledol mewn anhwylderau'r system weledol. Yn benodol, mae gennyf ddiddordeb mewn monitro dilyniant colled weledol, gan ddefnyddio technegau swyddogaethol a delweddu. Mae astudiaethau blaenorol wedi gwerthuso'r defnydd o ficroberimetreg mewn AMD ac mewn clefydau eraill, gan ganolbwyntio ar ddehongli clinigol mewn perthynas â data normadol.
Trosolwg addysgu
Mae fy nyletswyddau addysgu yn cynnwys rôl cyd-arweinydd modiwl ar gyfer y modiwl "Patholeg a Rheolaeth" ar gyfer myfyrwyr israddedig ail flwyddyn. Mae gen i awydd cryf i ymgymryd ag ymchwil sy'n cael effaith uniongyrchol ar gymdeithas. Rwy'n ymwneud â nifer o brosiectau ym maes iechyd y cyhoedd ac mewn prosiectau clinigol sy'n gysylltiedig â chlefydau retinol, megis dirywiad macwlaidd sy'n gysylltiedig ag oedran (AMD). Rwy'n aelod o'r Grŵp Iechyd Cyhoeddus Offthalmig a'r Grŵp Ymchwil Macwlaidd.
Dull ataliol ar gyfer Mynediad at lwybr System gyfan Cynaliadwy ar gyfer pobl hŷn ag ataliad vISion (ASSIST)
Bydd y prosiect yn mynd i'r afael â'r angen am ofal a chymorth mwy cynaliadwy i bobl hŷn sydd â nam ar eu golwg i'w helpu i fyw bywydau hapusach a mwy boddhaus. Gall y broblem i lawer o bobl hŷn â nam ar eu golwg fod yn cael mynediad at y math cywir o gymorth ar gyfer eu hanghenion unigol a dod o hyd iddynt. Mae Presgripsiynu Cymdeithasol yn golygu "cysylltu pobl â chefnogaeth gymunedol i reoli eu hiechyd a'u lles yn well", lle gall gweithwyr proffesiynol (e.e. meddygon, optometryddion, gweithwyr cymdeithasol, swyddogion tai) gyfeirio pobl at wasanaethau a gweithgareddau cymunedol.
Nod y prosiect yw darganfod beth yw'r ddarpariaeth bresennol o ofal a chymorth i bobl hŷn â nam ar eu golwg, i ddatgelu bylchau mewn mynediad a nodi meysydd arfer gorau. Byddwn yn argymell sut y gellir gwneud gwelliannau i gefnogi'n well, gweithio cydgysylltiedig a ffyrdd mwy effeithlon o ddefnyddio adnoddau presennol. Bydd y prosiect yn cynnwys dulliau cymysg, gan ddefnyddio dull realaeth i glywed barn unigolion â nam ar eu golwg a'u gofalwyr, yn ogystal â staff a rhanddeiliaid eraill.
Ysbyty i Gymuned: Gwerth optometryddion Golwg Isel cymunedol i randdeiliaid sy'n darparu ardystiad o nam ar y golwg mewn achosion o Ddirywiad Macwlaidd sy'n Gysylltiedig ag Oes Sych
Tan yn ddiweddar, roedd ardystio claf â nam ar ei olwg (CVI) yn gofyn am apwyntiad gydag Offthalmolegydd ymgynghorol yng Ngwasanaeth Llygad yr Ysbyty. Mae amseroedd aros ar gyfer apwyntiad gwasanaeth llygaid ysbyty ar ei uchaf erioed ac mae'n aros am apwyntiad i'r rhai sydd angen CVI yn tueddu i fod yn hirach oherwydd eu bod yn cael eu hystyried yn llai brys nag ar gyfer y rhai sydd mewn perygl o golli golwg na ellir ei atal. Mae CVI yn caniatáu cofrestru gyda nam ar y golwg gydag awdurdod lleol i gael mynediad at daliadau lles posibl gwasanaethau cymorth. Gweithredodd Llywodraeth Cymru newid polisi ym mis Medi 2023, fel y gall optometryddion arbenigol golwg isel ymgymryd â'r broses CVI ar gyfer pobl ag AMD sych yn y gymuned, naill ai yn eu cartrefi neu arferion optometreg. Bydd y prosiect hwn yn asesu effaith a chanlyniadau gofal iechyd gwerth ychwanegol economaidd optometryddion cymunedol sydd â chymwysterau uwch mewn golwg isel yn ardystio pobl â nam ar eu golwg oherwydd AMD sych.
Prosiect gofal llygaid acíwt
Mae'r prosiect hwn yn gydweithrediad amlddisgyblaethol gyda'r Athro Barbara Ryan, yr Athro Rachel North, Dr Angharad Hobby, Dr Andrew Carson-Stevens o'r Ysgol Meddygaeth, Dr Mat Smith o'r Ysgol Fferylliaeth, a Dr Nik Sheen o AaGIC, ac mae'n cael ei ariannu'n fewnol. Rydym yn ymchwilio i'r amodau llygaid acíwt sy'n cyflwyno i optometryddion, fferyllwyr, meddygon teulu ac i adrannau damweiniau ac achosion brys. Mae'r prosiect hefyd yn rhan o gydweithrediad â SAIL (Secure Anonymised Information Linkage), lle rydym yn cynnal dadansoddiad data ledled Cymru o ragnodi ar gyfer cyflyrau llygaid acíwt.
Gwerthuso gwallau wrth ddarparu gofal i gleifion â phroblemau sy'n gysylltiedig â'r llygad
Wedi'i ariannu gan KESS2 East, cyd-oruchwyliwr yr Efrydiaeth PhD hon yw Dr Andrew Carson-Stevens a'r partner diwydiannol yw Optometreg Cymru. Nod y prosiect hwn yw gwerthuso diogelwch cleifion sy'n gysylltiedig â'r llygad. Y prif becynnau gwaith yn y prosiect yw: Datblygu a phrofi system adrodd digwyddiadau diogelwch cleifion ar gyfer optometreg yng Nghymru; Dadansoddiad dulliau cymysg o adroddiadau digwyddiadau diogelwch cleifion yn disgrifio gofal llygaid anniogel yng Nghymru a Lloegr; Consensws rhanddeiliaid ar natur gofal llygaid anniogel y gellir ei osgoi.
Rhagnodi annibynnol mewn optometreg
Mewn cydweithrediad â'r Athro Molly Courtenay, Angela Whitaker ac ymchwilwyr o Brifysgol Swydd Bedford ac Ysgol Feddygol Bryste, gwnaethom astudiaeth ansoddol yn ddiweddar ar y ffactorau sy'n effeithio ar ragnodi gan optometryddion rhagnodol annibynnol, yn seiliedig ar y Fframwaith Parthau Damcaniaethol. Mewn gwaith parhaus ar y cyd â Dr Nik Sheen o AaGIC, rydym yn gwerthuso ymyrraeth i gefnogi optometryddion rhagnodol annibynnol sydd newydd gymhwyso.
Prosiectau eraill
Mae fy niddordebau ymchwil hefyd yn cynnwys archwilio swyddogaeth weledol yn AMD, a sefydlwyd yn ystod fy PhD ac a ddatblygwyd yn ystod cymrodoriaethau ymchwil ôl-ddoethurol rhyngwladol. Roedd gwaith blaenorol yn cynnwys asesiad topograffig y retina gan ddefnyddio delweddu amlfoddol mewn perthynas â swyddogaeth weledol a fesurwyd gan ficroberimetreg a thechnegau maes gweledol eraill mewn AMD ac mewn clefydau retinaidd eraill. Mae astudiaethau diweddar wedi gwerthuso cymhwyso data normadol mewn microberimetreg, gan ddefnyddio'r Asesiad Uniondeb Macwlaidd (Centervue); Ffigur 1) ac MP-1 (Nidek) offerynnau, i alluogi dehongliad cadarn o ganlyniadau yn y rhai ag anhwylderau system weledol. Astudiaeth arall yw gwerthuso perthynas microperimetreg ag offerynnau bywyd sy'n gysylltiedig â gweledigaeth ac iechyd mewn unigolion ag AMD.
Mewn ymchwil arall, rydym wedi penderfynu ar y cytundeb rhwng optometryddion golwg isel ac offthalmolegwyr ymgynghorol yn y broses o ardystio ar gyfer nam ar eu golwg. Rydym hefyd wedi amcangyfrif effaith adfer golwg ar ganlyniadau ansawdd bywyd unigolion â golwg isel. Mae ein canfyddiadau'n darparu cefnogaeth ar gyfer effeithiolrwydd y gwasanaeth adfer golwg.
Crynodeb o'r canfyddiadau
- Dangosodd optometryddion golwg isel ac offthalmolegwyr ymgynghorol berfformiad cyfatebol wrth ardystio nam ar y golwg, wrth ddosbarthu achosion cryno ar gyfer ardystio.
- Mae adfer gweledigaeth yn cael dylanwad cadarnhaol ar ansawdd bywyd sy'n gysylltiedig â gweledigaeth.
- Yn AMD cynnar, teneuo'r haen segment allanol, mesurwyd gan ddefnyddio segmentu haen retinol o ddelweddau tomograffeg cydlyniant optegol sbectr-parth, yn gysylltiedig yn sylweddol â sensitifrwydd microperimetreg is.
- Mae microberimetreg fasnachol wedi'i gyfyngu gan ddiffyg dadansoddiadau tebygolrwydd cadarn. Datblygwyd cronfa ddata normadol a model dadansoddi cadarn i ganfod annormaledd.
- Er gwaethaf tebygrwydd mewn gweithdrefnau ysgogiadau a throthwy rhwng microperimetreg ac ymylon confensiynol, mae gwahaniaethau pwysig rhwng offerynnau y mae angen eu hystyried wrth ddehongli canlyniadau meysydd gweledol. Mae'r rhain yn cynnwys y gwahaniaeth mewn ystod ddeinamig, data normadol a lefelau ymoleuedd cefndir. Roedd faint o golled weledol a ganfyddwyd, yn wahanol rhwng offerynnau ac roedd hyn yn dibynnu ar lefel addasu a math o glefyd retinol.
